As a Dependency Case Manger (DCM) you will support our growing needs for child case management services. You'll become a part of a strategic care team charged with attending to the needs of children placed in care by the Department of Children and Families, with the end goal of finding permanent, stable, safe home environments for children.

Responsibilities

YOU WILL:

  • Assess child and family needs to create referrals and linkages to services such as subsidy and medical coverage.

  • Facilitate removal and placement of children into safe and stable care while also supporting the families and care givers during the process.

  • Develop and maintain quality case and program written documentation that reflects client progress and also meets CHS, funders, and DCF requirements.

  • Travel to conduct home visits to access children's living environments and provide recommendations to their families and caregivers in a supportive manner.

  • Testify at court by providing client case status reports and recommendations regarding reunification, termination of parental rights or other permanency plans.

  • Build relationships with CHS team members and community partners in order to help facilitate discussions and meetings regarding case needs and recommendations

  • Provide on call support required as scheduled

Qualifications

YOU HAVE:

  • Strong time management skills and the ability shift priorities to meet outcomes

  • Ability to maintain professional boundaries and handle confidential information appropriately.

  • Exceptional interpersonal and communication skills that allow you to work well with diverse teams, clients and families.

  • A sincere passion for helping others in need.

YOU NEED:

  • Bachelor's Degree in a Human Services related field

  • Florida Driver's License within 30 days from hire

  • Possess the State of Florida Child Welfare Certification

  • Completion of the CHS Dependency Experience Waiver learning path unless the employee possesses one of the below preferred qualifications

  • PREFERRED:

  • Master's degree in Social Work

  • Two years experience working with at risk children and families

Please note that to be eligible for the sign-on bonus, you must be a Child Welfare-certified dependency case manager through the Florida Certification Board who is not a current employee of CHS, nor has been an employee in the last 180 days. Rehired team members are not eligible if they were last employed with CHS less than 180 days ago. A one year commitment is required as part of the sign-on bonus. The Medical Case Manager (MCM) blends the mission of AHF with Medical Case Management, utilizing their experience, skills and education together with a thorough understanding of the Ryan White Care Act Program within their specific area. With a caring and knowledgeable focus, you will work with our providers and healthcare center team so that every Ryan White patient receives the support they require and access to available resources. This will include providing education and assistance so that eligibility for the program is understood. Working directly within the Healthcare Center, you will meet with patients and complete Health Risk/Needs Assessments, develop Service/Action/Care Plans, and provide ongoing education to patients so that they understand their diagnoses and their medications. For Ryan White contracts that combine psychosocial needs within Medical Case Management, you will understand mental health needs and basic needs such as housing, food, and clothing and develop relationships with programs and agencies serving those needs and facilitate referrals, support and assistance for your patients who lack those services. By utilizing your case management experience, you will meet our patients where they are, identify strengths and opportunities for growth and assist them with realistic goal setting. With your skills of collaboration and engagement, you will bring the essence of the Ryan White Care Act - the safety net for the uninsured HIV/AIDS patients - to our most vulnerable population. By establishing Health Risk/Needs Assessments, Action/Service/Care Plans, referrals and support, your work will directly contribute to AHF's success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services. The MCM is often the liaison between provider and patient and is able to facilitate the shift from leaving the exam room to implementing the optimum plan of care and putting it into action. Providing brief interventions focused on a long term achievable goal, the patient and the MCM work together each step of the way towards viral suppression, better health outcomes and quality of life.
