Department Summary
HSC Health is the academic clinical practice of the Health Science Center. Focused on improving health outcomes to build a healthier community, HSC Health primary and multispecialty care clinics are located on the HSC campus located in Fort Worth's museum district.
Our care teams extend to local hospitals, surgery centers, long-term and continuing care centers, and mobile and in-home care. HSC Health's interprofessional teams work collaboratively to achieve an optimized treatment plan tailored to the individual patient. Physicians, nurse practitioners, physician assistants, physical therapists, pharmacists, social workers, psychologists, specialized technicians, and nurses work together while intentionally integrating healthcare students, residents, and fellows to deliver care to our community.
Position Overview
The Dementia Care Coordinator plays a pivotal role in identifying and addressing the ongoing care needs of our patients with dementia. Utilizing various data sources such as EMR and payer reports, the coordinator identifies gaps in care and facilitates the provision of necessary services to the patient population as part of whole person care.
This role also encompasses administrative duties for the GUIDE model and coordination of the dementia training and digital health project. Including;
Collaborating and coordinating project activities with partnering departments, institutions, and organizations.
Organizing and facilitating project meetings, including agenda creation, minute-taking, scheduling, and follow-up on action items.
Supporting the Project Director and clinicians in achieving project and patient care goals and deliverables.
Assisting in patient recruitment efforts and contributing to the development of recruitment and patient materials.
The Dementia Care Coordinator's role is comprehensive, ensuring effective coordination of care and successful implementation of the Guide model, other project and direct patient care objectives.
Minimum Qualifications
Bachelor's degree and three (3) year related experience; or equivalent combination of education and experience.

Preferred Qualifications
LMSW preferred with at least 2 years experience working with older adults
