POSITION PURPOSE

The Delivery Manager is responsible for managing delivery and transportation for our flatbed distribution center, including inbound truck, rail, store delivery, and last mile customer delivery for our flatbed DC in Dallas, TX. This position will support deliveries in the region, manage an on-site fleet, productivity, rail operations, yard operations, routing, capacity, store delivery & customer delivery.

MAJOR TASKS, RESPONSIBILITIES AND KEY ACCOUNTABILITIES -
25% Manage carrier relationships and planning function to ensure timely deliveries to stores while ensuring cube utilization; manage inbound deliveries to ensure on time, on cycle and lead time; manage inbound deliveries to ensure product is available for delivery to stores; manage outbound to minimize expedites; communicate daily with carriers to ensure on time compliance.
25% Manage cost of transportation; manage cost per truckload, cube per truckload, and cost per mile; support overall financial goals; fully utilize transportation assets to reduce freight costs whenever possible; audit invoices of carriers on regular basis; review general ledger for all charges dealing with transportation.
20% Drive Customer Service; develop customer service team; manage customer service associates and store concerns from phone calls and Infodock.
15% Financial and Operations Metrics development and reporting.
15% Maintain accurate operation of the yard integrity, dock, security, yard switchers and pallet recycling.

NATURE AND SCOPE -

Reports to DC General Manager I/II. Accountable for direct supervision of the work activities of others. Responsible for personnel issues -- including selection, termination, performance appraisal and professional development of subordinates.

EDUCATION REQUIRED -

The knowledge, skills and abilities typically acquired through the completion of a high school diplomas and/or GED.

Requires intermittent periods during which continuous physical exertion is required, such as walking, standing, stooping, climbing, lifting material or equipment, some of which may be heavy or awkward.

Ability to work a flexible schedule. Ability to be on-call at various times.

PREFERRED QUALIFICATIONS -

Bachelors' Degree concentrating in Operations Management, Business or Supply Chain Proficiency in Microsoft Outlook, Word and Excel software applications Extensive Transportation Background including TMS intelligence, Routing, P &L and Budgeting experience Previous Change Management Experience

They will interact and collaborate with interdisciplinary care team (IDT), which includes physicians, inpatient case managers, care team associates, pharmacists, social workers, educators, health care coordinators/managers. The Case Manager also acts as an advocate for members and their families linking them to other IDT members to help them gain knowledge of their disease process(s)and to identify community resources for maximum level of independence. The Case Manager will participate in IDT conferences to review care plan and member progress on identified goals and interventions. The Case Manager may perform telephonic and/or face-to-face assessments. Primary Responsibilities: * Independently conducts initial assessments within designated timeframes for members identified as having complex case management needs (assessment areas include clinical, behavioral, social, environment and financial) * Collaborates effectively with IDT to establish an individualized care plan for members with interventions to assist the member in meeting short- and long-term goals * Engage patient, family, and caregivers to assure that a well-coordinated care plan is established * Identifies member needs, develop care plan and prioritize goals; using evidence-based practice the Case Manager will develop interventions while considering member barriers independently * Make outbound calls to assess member health status, identify gaps or barriers in care plan * Provide member education to assist with self-management goals * Make referrals to outside sources * Educate members on disease process or acute condition and provide indicated contingency plan * Coordinates visits with PCP and specialists as needed on a limited basis, performs visual assessment for skin checks on exposed skin, and conducts home safety evaluation * Independently confers with Market MD Medical Directors and/or PCPs on a regular basis regarding Complex Care/High Risk Population members and participates in department huddles * Enters timely and accurate data into designated care management applications and maintains audit scores of 90% or better on a monthly basis. * Adheres to organizational and departmental policies and procedures * Takes on-call assignment as directed * Maintain current licensure to work in state of employment. Decision making is based on regulatory requirements, policies and procedures, and current clinical guidelines * Maintains current knowledge of health plan benefits and provider network including inclusions and exclusions in contract terms * Monitors for quality concerns regarding member care and reports as per policy and procedure.
