Child First (Child and Family Interagency Resource, Support, and Training) is an evidence-based in home early childhood intervention, embedded in a system of care that works to decrease the incidence of serious emotional disturbance, development and learning problems, and abuse and neglect among the most vulnerable young children and families. The Data Specialist will be responsible for supporting continuous quality improvement, ongoing monitoring and audits of data to ensure fidelity to Child First model, Child First benchmarks and Accreditation standards. Support outreach efforts and provide limited administrative supports to clinical coordinator.

Helps Clinical Coordinator to plan, design, implement, and maintain a comprehensive continuous quality improvement (CQI) program to ensure fidelity to the Child First model. Assists in educating staff about QI systems and requirements.

Meets with Clinical Coordinator and internal audiences to identify and problem solve QI issues.

Monitors program efforts to ensure compliance with Child First benchmarks, accreditation standard measures, client satisfaction surveys, and reports on results.

Performs audits of electronic medical records (CFCR) and reviews other data documents/systems (ASD, etc.) to ensure complete and accurate documentation.

Helps to prepare various monthly and quarterly reports.

Creates and manages program outreach materials, may include community presentation materials.

Records and maintain group supervision meeting notes including agenda, discussions, decisions, and follow ups.

Attends meetings and engage in trainings as needed to perform duties and support program needs.

Performs other duties as assigned or as necessity dictates.

Qualifications

  • Bachelor's degree

  • Experience with timely and accurate data collection and entry and providing outcome reports.

  • Experience working in early childhood system of care within North West Region of CT.

  • Knowledge and/or experience with providing in home services for young children and their families.

  • Knowledge and experience working diverse cultures and ethnicities

  • Knowledge of continuous improvement principles

  • Ability to understand and use analytical reasoning to interpret data

  • Excellent oral and written communication skills

  • Exceptional listening and analytical skill

  • Highly organized, self-motivated, and flexible,

  • Strong computer skills especially in Word and Excel

  • Strong commitment to the vision, mission, and goals of Child First.

Job Responsibilities * Provides clinical support and case management services to support physicians and APRN's in Outpatient Behavioral Health setting. * Respond to telephone requests for medication; collect medical information related to refill requests; verify medication requests; place orders for providers related to medications; e-prescribe/call in medication refills for non-controlled substances with a providers order; and inform patients of normal test results. * Fulfill patient care responsibilities as assigned that may include: scheduling appointments; checking provider schedules; facilitating timely patient access and clinical flow; accompanying patients between behavioral health units, the lab, or the emergency department; assisting patients as needed with walking; toileting; collecting urine specimens, preparing for provider sessions; ambulating to/from transportation; placing calls to family/agency support; collecting patient history; performing screenings per provider guidelines; assisting physicians/nurses with various approved procedures; charting; scheduling appointments; verifying insurance; medication authorizations; relaying instructions to patients/families; answering calls, and providing pertinent patient resource information. * Assist providers with various patient support responsibilities that include: setting up provider rooms, obtaining forms/materials for providers; cleaning provider/nursing rooms and equipment between patient visits to maintain infection control; and inventory control/ordering/stocking of supplies needed by providers; administration of patient screening instruments for providers; collects and enter patient data as assigned by the Clinical Manager. * Provide support to patients as indicated while they are waiting to be seen for appointments.
