Within a collaborative team environment is responsible for higher level Health Information Services responsibilities associated with the integrity of patient data, completion of new born birth documents, duplicate and medical record overlay analysis, updates to the Physician Registry data base, provides subject matter expertise to Living Wills and Health Care Proxy documentation, provides oversight to Vendor physician dictation and transcribed documents and support to HIS Document Capture colleagues when required.

Reviews, analyzes and when appropriate corrects duplicate and/or overlay medical record number assignments which represent a patient#s primary identifier, all encounters aggregate to, and if in error, visits are moved to the correct patient, and encounters may need to be combined in an accurate, timely manner to ensure integrity of primary and secondary patient identifiers impacting safety, risk (legal medical record) and finance. (billing).# Reprocessing electronic and scanned medical record documentation, inclusive of prepping, scanning quality control and validation must also occur. Completes all documents associated with Newborn births/deaths at Saratoga Hospital inclusive of birth certificate, clinical birth data, Social Security identifier (number) assignment and Parentage Papers when appropriate.# Collaborates with City, State, and Federal agencies, Clinical staff in addition to the Saratoga Spring community.# Reviews and makes medical record number corrections based on minimal and unclear data received via email and phone calls from various Saratoga Hospital departments communicating the need for patient identity review/analysis. Updates and maintains the Physician Registry database inclusive of Saratoga Hospital Privileged, Not in Dictionary, and Unknown practitioners to ensure hospital claims bill timely, accurately and compliantly. Provides subject matter expertise to internal and external customers regarding the validity of a Living Will, Health Care Proxy, and how to navigate/locate in the Saratoga Hospital electronic health record, and when validity has been ensured, documents are entered/scanned in a timely manner for reviewing parties.# Ensures patient demographic information is accurate in the Saratoga Hospital, Master Patient Index data base inclusive of Patient#s Name, Date of Birth, Address, Social Security Number and Sex, and following verification will make correction to ensure the integrity of demographic data. Reviews Data Integrity reports, lists and queues timely and accurately. As a back-up, and/or in collaboration with HIS Application#s Analyst, ensures all transcribed documents upload to the electronic health record and those with quality issues are reviewed and corrected to ensure continuity of patient care and electronic health record accuracy.# Provides support to Document Capture for prepping, scanning quality control, validation during period of volume increases and/or staffing challenges. Provides support and cross coverage to HIS Application#s Analyst and Form#s Coordinator to ensure smooth Health Information Services operations.
Meets performance expectations for customer service, teamwork, resource utilization and development. Performs other duties as assigned or directed to ensure smooth operations of the Health Information Services department/unit. Minimum Qualifications: Education, Training # Experience
6-8 years of experience in a health care setting.# Associate degree with 2-4 years of health care experience preferred. Certifications / Licensure
