The Substance Use Treatment Community Support Specialist (CSS), working in both center-based and community settings, will engage members in treatment through outreach, counseling and coaching, to help members establish and work toward recovery goals.
The Substance Use CSS will also provide assertive outreach to those who have disengaged in care, as well as community members who are not in treatment but are at risk for substance use disorders. In addition, the Substance Use Treatment CSS will provide follow-up and aftercare for members who have completed treatment.
ESSENTIAL DUTIES & RESPONSIBILITIES
Engage members in recovery and rehabilitative services through harm reduction, motivational interviewing, and motivational enhancement techniques.
Work with members to develop a Continuing Recovery Plan that addresses their comprehensive needs.
Link members to recovery support services, including facilitating the use of community-based mutual aid resources.
Assist members in connecting with an external provider for continued Medication Assisted Treatment.
Facilitate alumni support groups.
Routinely meet with members in their community environments.
Provide support to members with collateral sources, including other agencies.
Collaborate with members to engage family and other social supports in recovery planning.
Accompany members to meetings, appointments, etc., in the community, using public transportation.
Provide overdose prevention education and support.
MARGINAL DUTIES & RESPONSIBILITIES
Perform and participate in crisis intervention when necessary.
Provide accurate and timely information to the supervisor on all topics and issues related to the operation of the agency and the well-being of members.
May be required to work flexible schedules and be available to work on weekends or evenings as required.
Substance use and mental health recovery counseling and assessment experience.
Experience working with persons diagnosed with substance use and mental health needs preferred.
Ability to work as an effective team player.
Ability to document services provided in an Electronic Health Record in a timely manner
Strong interpersonal skills
Experience within a hospital or medical setting preferred.
Knowledge of medical terms and ability to communicate with medical staff is strongly pref
Professional staff providing clinical services shall have at least one of the following:
Certified Alcohol and Drug Counselor (CADC) + 2 year's experience
Licensed Professional Counselor (LPC)
Licensed Social Worker (LSW)
Licensed Clinical Social Worker (LCSW)
Licensed Clinical Professional Counselor (LCPC)
