Qualifications: Bachelor's or Master's degree in human services or independently licensed as a LICSW or LMHC and one (1) year experience working with the target population. Must be certified in the Massachusetts CANS. Experience in navigating child/family service systems and experience advocating for family members who are involved with behavioral health systems. Substantial direct experience working with child mental health issues and crisis management. Experience with, and commitment to, family-focused care for children; demonstrated capacity to work collaboratively and effectively with families and community-based colleagues (teachers, caseworkers, informal supports) toward the normalization of children's functioning. Strong interpersonal skills in terms of developing a working relationship with a variety of individuals in a variety of contexts. Ability to tolerate ambiguity and use initiative within the framework of the mission and scope of a project. Consistent with licensure level and required experience, the ability to perform other Commonwealth Children's Behavioral Health Initiative services as directed according to the specifications established by the CBHI. Computer literacy and skill in the use of computers and related software applications. Ability to consistently meet all necessary background and credentialing requirements. Must have valid drivers license and reliable transportation to and from work and for providing services in the community. Must have reliable telephone notification system for emergency contact.

Must be proficient in English and possess and convey excellent verbal and written communication skills with clients, staff and service providers. Demonstrate sound judgment and effective, solution focused, problem solving skills. Ability to develop and maintain professional working relationships with clients, staff, co-workers and supervisor. Ability to work effectively with diverse populations and cultural groups.

Physical and Mental Demands: Frequent walking, standing and driving to carry out job responsibilities. Ability to work a flexible work schedule contingent upon program needs. Ability to work effectively in a fast paced environment with periods of high stress.

Responsibilities:

1.Serve as key contact for the youth and family to the Eliot CSA and its services. Hold primary responsibility for initiating care planning activities.

2.Within 24 hours, provide phone outreach to new referrals regarding the Eliot CSA and the role of the Care Coordinator and Family Partner.

3.Initiate planning that includes intake meetings offered within 3 days after referral, to hear directly from the family what they hope to achieve through involvement with Eliot CSA, orient the family and the member to the wraparound process, identify strengths and needs of the child and family, and begin development of the wraparound team membership.

4.Conduct a comprehensive home-based assessment inclusive of the CANS and other tools as determined necessary, which occurs in the youth's home or another location of the family's choice.

5.Develop a risk management/safety plan in collaboration with the family and collaterals.

6.Develop and implement a youth and family centered ICP in collaboration with the family and collaterals.

7.Make referrals and linkages to appropriate supports as identified in the ICP.

8.Identify and develop natural family supports.

9.Identify and assist the youth and family to obtain and monitor the delivery of available services including, medical, educational, social, therapeutic and other related services. Assist family with system navigation of these services.



  1. Conduct and facilitate monthly Care Planning Team meetings in accordance with contractual expectations for membership. Monitor and authorize on-going use of services provided to members to meet the specific goals of the member's plan, as identified within the CPT. Work with the CPT to identify or develop informal community resources to support the member's accomplishment of their goals.

  2. Facilitate ongoing discussion regarding the short-term nature of CSA services and transition planning. Conduct monthly reviews with CPT using established Transition Indicator Tools for both Youth and Caregiver/s.

  3. Fulfill all responsibilities as HUB of CBHI services. During transition planning, work in collaboration with youth, family, and other CBHI services to ensure that a new HUB and HUB dependent services provide/receive all required paperwork and understand all responsibilities.

  4. Monitor, review and update the ICP to reflect the changing needs of the youth and family.

  5. Document Care Coordinator activity on behalf of specific goals in member's Daily Progress Note. Review Crisis Plan weekly and make revisions when needed. Complete all other regular documentation as required in a timely manner.

  6. Collaborate effectively with families and others involved with the Eliot CSA.

  7. Work in concert with the Family Partner to provide family education, advocacy and support.

  8. Maintain a minimum of weekly contact with the family, youth and other relevant persons in the youth's life to hear how they feel the plan is progressing and to respond to concerns they may have.

  9. Maintain face to face contact with the youth and family as determined by the youth, family and members of the CPT.

  10. Initiate contact with others involved with the youth and family that:


a. Supports their understanding of the child and families experiences.

b.Supports their expression of their understanding of the member's needs and their role in helping the family to meet those needs.

c. Supports their capacity to identify commonality with family members (or other involved parties) to facilitate goal clarification and attainment.

d.Supports their capacity to identify difference with families members (or other involved parties) and ways to constructively address and resolve those differences.



  1. Carry a cellphone during the day for contact by other staff or collateral workers. Support the family and member in the implementation of a Crisis Plan when needed. Notify the Advanced Care Coordinator, Senior Care Coordinator and/or Program Director when there is a clinical crisis or any change in child's placement.

  1. Provide on-call coverage as defined in on-call procedure, including responding to the on-call answering service messages. Provide case coverage for other Care Coordinators, arranged in advance, to a standard, which supports progress in the member's care plan.

  2. Attend weekly individual and group supervision meetings with the Advanced Care Coordinator or Senior Care Coordinator, as well as CSA staff meetings and contribute effectively to the purpose and tasks of these meetings.

  3. Meet established productivity target.

  4. Enter all documentation and billing requirements into the electronical medical record within established deadlines.

  5. Update and maintain client's electronic medical record in accordance to contract specifications.

  6. Obtain and maintain proper insurance authorization for client service coverage.

  7. Provide transportation when there is a clear purpose and intent to support treatment goals.

  8. Comply with all statutes and regulations relevant to the maintenance of clinical standards in the Commonwealth, e.g., mandated reporting, and duty to warn.

  9. Obtain and maintain current certifications in CPR, First Aid, and all other required training. Ensure that all certificates are current and provide required documentation to the Agency.

  10. Responsible for promoting and upholding the principles of Corporate Compliance and the ECHS Corporate Compliance program. Adhere to all ECHS policies and procedures including the Code of Conduct, Consumer Confidentiality, and HIPAA.

  11. Act in an ethical, legal, and responsible manner, following all applicable ECHS policies, state/federal laws, regulations, and reporting requirements.

  12. Respect diversity of others by demonstrating and modeling cultural awareness and sensitivity at all times.

  13. Perform other job related duties as assigned.
