Einstein campuses are Tobacco and Smoke Free.In this role you will be responsible for:Working in collaboration with the physician to provide consultation in palliative care, symptom management, goals of care conversations and supportive care to meet the complex medical needs of the patient as requested by the referring physician

Facilitates clarification of patient and family goals of care, provides symptom management, coordination of care across the continuum with facilities and families, and facilities access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment)Assists with diagnosis, treatment and complex management of acute and chronic health conditions impacting patient quality of life

Support the hospice program at times for face to face visits

This position requires providing service to the following age population(s) adult, geriatric and frail elderly in a manner that demonstrates an understanding of the functional/developmental age of the individual served

If you possess the following qualifications, please apply immediately:Successfully completed an accredited board approved masters or post-masters practitioner program and will have training and experience commensurate with the expectations of the Medical Director It is preferred that he/she will have experience in outpatient medical care or specialty

Maintain active unrestricted CRNP and DEA license in Commonwealth of PACurrently certified and maintain certification as a Certified Registered Nurse Practitioner (CRNP) from a board recognized national certification organization which requires the passing of a national certifying examination in the hospice and palliative care specialty area

Valid Pennsylvania Drivers LicenseCurrent automobile insurance and reliable vehicle for use on duty


Employment is contingent upon review and recommendation for approval by the Physician Companies Credentialing Committee and final approval by the Physician Companies Quality and Compliance CommitteePhysical Demands:Physical

Requirements:
Standing, walking, lifting and moving of patients and equipment

Visual Requirements:
Must pass color perception test

Hearing Requirements:
Normal hearing

Working Conditions: High volume area
