DUTIES: Coordinates a patient-focused team in providing direct and collaborative health care in a variety of clinical settings designed to promote and restore optimal health across the life span.

Utilizes all components of the nursing process in providing care that reflects an understanding of individual cultural and religious diversities and needs, and recognition of patient rights. Utilizes knowledge and expertise in case management to provide comprehensive, coordinated, seamless care to individuals and aggregate groups. Coordinates, integrates and facilitates the patient/provider relationship along the continuum of care.

QUALIFICATIONS: Graduate of an accredited school of nursing. Bachelors Degree preferred. Current licensure as R.N. in New York State.

ANA certification or certification from appropriate agency in specialty preferred. One year med/surg experience and one year specialty experience preferred. SPECIAL REQUIREMENTS:

Ability to operate all equipment found in care center as necessary to perform all prescribed duties. Ability to access and navigate clinical computer system. Ability to communicate in a tactful, diplomatic way.

Ability to initiate projects and to carry out day to day operations with a minimum of supervision. Ability to handle information with strict confidentiality. Excellent written and verbal communication skills.

Despite the developments that have occurred over the years, the original commitment remains the same--from the moment the staff enter a home until the last contact with that patient, personnel will endeavor to offer the most comprehensive and compassionate health care available. Catholic Home Care manages home care services for approximately 2,000 patients daily across Long Island and Queens. Specialized services include skilled nursing care, physical and occupational therapies, medical social work, maternal-child and pediatric services, home health aide assistance, nutritional counseling, pastoral care, infusion services, telehealth, palliative care, wound care and behavioral health services. The RN Care Manager is responsible for the care planning, utilization of services, coordination of care through weekly conferences, and the education of the teams in the management of patient care services, providing patient centered care efficiently, effectively, and with the best possible outcomes. Implements best practices and evidence based standards of care, moving toward standardization of clinical care for all disciplines. The RN Care Manager approves/disapproves all patient care plans, new and revised, and acts as the case manager between the teams and insurance companies who require authorization of services. RN CARE MANAGER Responsibilities will include: * Conducts weekly case conferences with the teams and maintains the Patient Utilization Profile for each patient. * Approves or disapproves plan of care; makes recommendations for efficient and effective utilization of resources to achieve positive clinical, quality and fiscal outcomes. * Obtains case management information from the nurses/therapists in a timely manner and processes requests for authorization of services, following initial authorization obtained in the Intake Dept. * Recommends additional services to improve efficiency: utilization of CWOCN, medical social worker, telemedicine/remote monitoring and therapies. * Promotes fiscally responsible care for agency patients, reviewing the clinical, functional and service utilization deficits/needs of patients with the team. Requirments: * Licensed and current registration to practice as a Registered Professional Nurse (RN) in New York State required. * Minimum of three (3) years post-licensure in nursing with experience in a CHHA. * Demonstrated leadership, organizational, communication and interpersonal skills required. * Must have strong technology skills Electronic Medial Record experience required.
