This position qualifies for a hire-on bonus of $1000 with 1/2 paid with 1st check after hire and 1/2 paid with 1st check following the 6-month probationary period.

Clatsop Behavioral Healthcare is seeking a full-time Crisis Systems Case Manager. This position receives client assignments from the Mobile Crisis Team, Forensics Team, and the Rapid Access Clinic - co-responding with the Mobile Crisis Clinicians and intervening with clients that are experiencing a crisis episode or involved with the legal system by providing intensive case management to help them address their needs from a holistic and comprehensive standpoint.

The Crisis Systems Case Manager will mainly work in the community, interfacing with clients at locations varying from Emergency Departments to correctional facilities to local emergency shelters. This position may also work from a designated CBH office as needed. The target population consists primarily of clients experiencing a crisis or involved in court processes such as the aid and assist program or participating in general jail diversion processes. The Crisis Systems Case Manager will work with clients at various levels in their case progression (e.g., on conditional release programs). This position serves as a CBH contact for efforts related to diversion from incarceration. The Crisis Systems Case Manager will be involved in outreach efforts and follow-up for crisis clients to support the goal of diversion from incarceration or entanglement in the legal or healthcare systems.

Client case management needs include but are not limited to, managing mental health challenges, barriers to accessing and engagement in services, physical health issues, accessing benefits and entitlements, and obtaining affordable housing. Clients are often not yet connected to necessary resources and may require assistance covering apparent service gaps. Many are diagnosed with co-occurring substance use disorders, severe mental illness, or other medical problems that present significant challenges in multiple areas of their lives.

Essential Duties and Responsibilities:

  • Participates in daily Crisis Team meeting.

  • Participates in ongoing case management, ongoing needs assessment, and support of their treatment plan

  • Participates in jail discharge planning as needed in support of conditional release requirements

  • Collaborates with natural and community supports

  • Provides services with a collaborative, solution-focused approach

  • Collaboratively manages a variable caseload of clients involved or at risk of involvement in legal system, e.g. clients with unresolved charges

  • Provides services in the community at various locations: jail, court, emergency shelters, CMHP building, Emergency Departments, etc.

  • Documents case updates for Crisis or other cases in Care Logic, and shares information with all parties concerned

  • Performs outreach and engagement as a preventative measure i.e. diversion from incarceration or from a Higher Level of Care (HLOC)

  • Meets with clients to assess needs and considers additional resource referrals both within and outside of CBH

  • Coordinates with CBH program administrator (e.g. Crisis Systems Program Manager, Crisis Systems Assistant Manager, Exceptional Needs Care Coordinator (ENCC), or other designated staff) to provide accurate updates on client engagement

  • Assists with referral and coordination of entry into diversion programs such as Jail Deflection, Mental Health Court, or Drug Treatment Court as needed

  • Assists with referral and coordination of entry into other programs within CBH as needed, when not duplicating services

  • Additionally provides in-person community outreach to clients and connection to natural supports and support providers of clients by visiting them in their homes, workplaces, or in the community

  • Changes method and degree of treatment when indicated by clients' reactions as outlined in the treatment plan

  • Discusses progress toward goals with clients, clients' families, and other natural support providers as appropriate

  • Regular consultation with other team members to ensure client needs are being met

  • Encourages use of identified coping skills, facilitates legal skills or other skills training groups, teaches life skills, and other evidence-based services

  • Refers clients to other supportive services to supplement treatment and counseling

  • Documents all services provided to participants according to agency standards and timelines

  • Consults and coordinates services with appropriate community partners such as: Emergency Department Social Workers, Developmental Disabilities, Primary Care Physicians, Department of Human Services, Senior & Disability Services, Probation & Parole, Community Action, schools, families, and natural supports

  • Obtains Releases of Information to be able to consult and coordinate care with other community agencies

  • Maintains client confidentiality and adhere to ethical standards of the profession

  • Participates in Investigations, court hearings, and other legal proceedings as directed by Supervisor and/or subpoena and maintains the required documentation for the same

  • Remains flexible to the changing demands of a mental health system and the needs of the agency

  • Coordinates with community-based Outreach workers in order to build rapport with the community-at-large, and to engage in preventative outreach of acute and vulnerable individuals.

  • Performs other duties as assigned.

Education and Experience:

  • Must have or have the ability to obtain within 6 months of hire the Qualified Mental Health Associate (QMHA) certificate.

  • Prior experience in the field of case management working with individuals in mental health or substance use treatment.

  • Knowledge of the DSM 5 & ASAM Levels of Care

  • Knowledge of processes related to mental health in Oregon (e.g., Aid/assist, civil commitment, etc.).

  • Must have general keyboarding and computer skills.

  • Must be able to read, write, and communicate in English.

  • Skilled in effective case management and care coordination

  • Skilled at remaining calm in stressful situations and de-escalating potential client aggression.

  • Skilled at group facilitation.

  • Successful completion of a background check.

  • Must have and maintain a valid Oregon or Washington Driver's License and meet the agency's automobile insurance carrier's standards.

Preferred

  • Bachelor's Degree in a behavioral science field Preferred.

  • CADC preferred but not necessary.

  • Experience working in corrections or the legal system is preferred.

  • Additional language skills in Spanish are desirable.
