Trenton Health Team (THT) is an innovative multi-sector partnership dedicated to the health and well-being of the greater Trenton community. Nationally-recognized for results achieved over more than a decade of work, the collaborative is an innovative partnership among St. Francis Medical Center, Capital Health, Henry J. Austin Health Center, the City of Trenton Department of Health and Human Services, and more than 100 community organizations.
THT operates with a broad understanding of what health, health equity, and well-being mean for Trenton, as well as what activities and strategies will help address all the factors influencing a healthy city. Our shared Vision for a Healthy Trenton incorporates social determinants of health, defining a healthy Trenton that includes: Healthy Environment and Housing; Vibrant Public Spaces; Growing Economy; Healthy Food; Improved Safety; Equity and Social Cohesion; and Quality Healthcare.
The COVID Relief Program is centered around expanding vaccination rates within the Trenton community. Throughout the pandemic, Trenton Health Team coordinated key stakeholders and healthcare providers to ensure collaboration and data sharing aimed at educating the public and driving down COVID transmission rates. THT will continue to serve as an integrator for our region, with responsibility to collaborate effectively with a wide range of stakeholders - both individuals and organizations, ranging from neighborhood residents to social service providers to governmental agencies and leaders.
The COVID Relief Community Ambassador is a member of the COVID Relief Team funded by a short-term grant provided by the State of New Jersey. This effort requires collaboration between THT and various partners such as THT Community Advisory Board (CAB) participants, resident organizations, Henry J Austin Health Center, Capital Health, St. Francis Medical Center, faith-based organizations, local businesses, Trenton Public Schools, the City of Trenton, Mercer County, and the State of New Jersey.
This temporary initiative focuses on three main areas of activity:
Reporting to the COVID Outreach Leader, the COVID Relief Community Ambassador is primarily responsible for connecting members of the local community with the programs and services offered in support of the COVID Relief program. Other related duties and responsibilities may be assigned. The Community Ambassador role requires flexibility, as assigned duties will often require work outside of normal business hours, and assigned duties will vary from day to day.

The COVID Relief Community Ambassador performs job duties in a normal business office environment in an urban community. Office tasks are generally carried out with no specific or unusual physical or environmental demands. While performing the office duties of this job, the COVID Relief Community Ambassador is regularly required to sit at a desk and work with a computer for extended periods of time. Specific physical abilities required by this job include operating basic office equipment such as personal computers, copy machines, and standard office telephones. The COVID Relief Community Ambassador also performs job duties in the Trenton community and at events held at various locations. Specific physical abilities required by this job include transporting and lifting event materials, and standing or walking for extended periods of time.
Approximately 75% percent of the time will be spent in the community and the COVID Relief Community Ambassador must be able to travel to and from various sites as needed.
While performing community tasks, the role may require:
Further, the COVID Relief Community Ambassador will be required to attend meetings that will occur in the evenings and/or on weekends both on-site and off-site, in-person or via phone or videoconference.
