Job Summary
The Counselor is a Clinician who assumes responsibility and accountability for a patient or group of patients for a designated time frame. Will conduct a variety of psychosocial assessments and provides care for these patients through therapeutic use of self and/or the patient care/problem solving process to assist the patient and family to achieve basic healthcare goals. The counselor is a member of an interdisciplinary team that is responsible for working collaboratively with the medical director & clinic manager to ensure development and implementation of appropriate standards. Needs to have flexibility in traveling between two hospitals 30 minutes apart.
Job Requirements
Education
Required: Masters Degree in a related field of counseling services
Experience
Preferred: 1-2 yrs. experience with wide variety of patient populations
Skills
Ability to work within scope of practice to help patient achieve highest level of quality possible for psychological well-being
Knowledge of community resources to help meet patient needs
Ability to work independently & collaboratively in teams
Licensure/Certification/Registration
Required: Licensed Professional Counselor (LPC), LMFT, or provisional license with completed degree requirements
