COVID-19 Considerations:

Locations and shifts are available throughout Delaware County


  • Schedules medical appointments for consumers.

  • Provides transportation for appointments, outings, etc.

  • Performs administrative duties, including accepting/delivering accurate messages, filing, etc.

  • Supervises, assists, and trains consumers in self-care and independent living skills, including bathing and all areas of personal hygiene.

  • Participate in the development and implement the ISP.

  • Assists consumers in food shopping, preparation, serving, cleaning and care of home, laundry, personal hygiene, menu planning and implementation, etc.

  • Documents consumer progress and other necessary documentation.

  • Ensures confidentiality of consumer records.

  • Performs related work as required.


  • Relevant training and/or experience in the field of Human Services (experience working directly with individuals with developmental and/or intellectual disabilities preferred).

  • Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record; otherwise-qualified candidate may be considered for non-driving position with less than three years of driving experience in the United States to be converted to a driving position upon completion of said three years


  • Minimum of High School Diploma or G.E.D.

VIEW JOBS 3/10/2021 12:00:00 AM 2021-06-08T00:00 The Counselor/Therapist will conduct behavioral health screenings and will refer enrolled youth and their families for additional services, as warranted, including behavioral health services such as individual therapy for the enrolled youth, or family therapy to address family issues. As a master's trained counselor, the Counselor also will conduct regular individual counseling sessions with each youth, as well as group counseling with other members of the BIPS community. Counselors will build on the Bridge's extensive knowledge of neighborhood health, social service, behavioral health, educational, faith based, and recreational/social socials services to coordinate and follow-up on specific goals established in the Academic Support Action Plan. The Counselor reports to the BIPS Program Director. Responsibilities: * Works as a team with the Therapist/Counselor in providing systemic treatment to adolescents and their families both on-site and in their homes using various professional integrative and collaborative modalities. * Assist with intake and assessment information using the Bridge bio-psycho-social assessment form and a recovery-oriented, culturally competent assessment approach. * Conduct behavioral health screenings on new clients or as issues emerge * Provide weekly family-focused counseling/case coordination to approximately 16 adolescents and their families, depending on each family's individual needs. * Participate in Service Plan meetings to develop discharge criteria and working in collaboration with the BIPS case manager. * Develop strong relationships with community-based organizations and resources throughout the city to help client and her family establish linkages to appropriate community resources. * Ensure that discharge planning is included in treatment planning and is the focus of case management from the time of admission. * Conduct psychoeducational, Life Skills, and other didactic groups for BIPS clients and family members. * Collaborate with representatives from other systems involved in the youth and family's treatment process, particularly behavioral health providers. * Participate in all required case consultation meetings, staff meetings and trainings. * Help develop ASAP plans with the case manager, family, youth and school personnel. * Adhere to all written documentation protocols as required by DHS * Provide referrals for support services when warranted. * Enhance, update and remain current with educational advances within the discipline and field by attending at least 25 hours of in-service or educational experience. * Attend staff meetings * Attend weekly supervision and provide agenda for these supervisory meetings. * Alert Program Director and Service Team of any concerns regarding clients that might jeopardize the safety of the program, client or other clients. * Other duties assigned by Program Director. Skills: * Competence in individual, group and family therapy with an emphasis on empowerment * Ability to use therapeutic skills essential to assisting at-risk youth. * Knowledge, understanding and experience working with at-risk youth, substance abuse, trauma and/or mental illness * Strong knowledge of community resources relevant to this population. * Knowledge and experience and ability to provide family partnership/parent training * Ability to utilize supervision to improve collaboration with treatment team and provision of family treatment * Sensitivity to racial, cultural, economic, and willingness to adopt interventions that work for different groups. * Understanding of a strengths-based, recovery-oriented approach and a willingness to adopt this approach. * Knowledge of high-risk youth, family systems, mental health disorders and substance abuse. Experience: * At least 2 years related experience. Education Requirement: * Master's Degree in social work, psychology, family therapy or related field required. * Valid driver's license and vehicle required.
