HHHunt's Spring Arbor Senior Living of Albemarle is seeking a qualified Cottage Care Director. The Cottage is HHHunt's memory care community. The Cottage Care Director is a unique opportunity for a licensed nurse who also enjoys flexing their creative skills! The ideal candidate will be a strong leader, will be clinically sound and will also be responsible for oversight of a person-centered, life-skills program, with the assistance of a Cottage Activity Assistant, for our memory care residents. The CCD is the subject matter expert on all things related to managing the care of our memory impaired residents. This role is an integral part of the leadership team. A LPN or RN license is required.



  • Nursing licensure, LPN or RN, required

  • Supervisory role of all Med Techs, CNAs and other cottage staff

  • Recruit, hire and manage cottage care staff

  • Complete scheduling for cottage care staff

  • Orient, monitor progress and reinforce training of cottage care staff, including state required training

  • Collaborate with Resident Care Director (RCD) and Executive Director in implementing state regulations as required

  • Complete all HHHunt and state required documentation in a timely manner

  • Function as part of the management team (manager on duty coverage, presenting the community, planning and participating in staff meetings, planning and implementing marketing and community events)

Programming, Resident Care and Nursing Responsibilities

  • Develop a care team and residential living environment that supports overall Cottage Care philosophy

  • Create and coordinate implementation of ongoing person-centered activities and life skills programming

  • Monitor physician orders to assure follow-through and completion

  • Audit Medication Administration Records (MAR) and notify pharmacy of changes

  • Complete resident assessments as outlined by HHHunt and licensure guidelines in a timely manner and train designated team members in assessment skills

  • Develop and implement resident care plans to meet the needs of residents per HHHunt and state policies

  • Monitor all aspects of the medication administration program

  • Direct and coordinate resident care service needs in collaboration with care team and community department heads as appropriate

  • Consult with families and/or physicians to coordinate proper resident care

  • Communicate with physicians and other medical professionals regarding residents' medications, treatments, and/or therapies

  • Develops and coordinates strategies to reduce future potential risk


  • Experience in and knowledge of diagnoses

  • Knowledge of medication and administration procedures

  • Knowledge in diagnostic review of resident falls

  • Knowledge in skillfully conducting resident assessments and creating appropriate Plans of Care

  • Educational background in understanding cognitive impairment

  • Ability to coordinate with medical providers with understanding of physiology

  • Ability to conduct professional care plan meetings with families as a licensed advocate for resident's needs

  • Knowledge of effective supervisory and management practices, techniques, and methods

  • Knowledge of resources for seniors and of related agencies

  • Understanding of the social, psychological, and recreational needs of Alzheimer's and memory impaired residents

  • Ability to exercise confidentiality following HIPAA guidelines

  • Pleasant, friendly, and helpful at all times ensuring proper and timely service

  • Genuine concern for and strong interest in working with seniors

  • Ability to demonstrate a positive attitude at all times

  • Must be 21 years of age or older

  • Experience or training in an equivalent setting preferred

  • RN or LPN required and in good standing with state licensing authority

  • General understanding of and concern for the needs of seniors

  • Aptitude and previous experience with Alzheimer's and memory impaired residents

  • Ability to work in an environment conducive to caring for residents without posing a substantial safety or health threat to self or others

  • Ability to manage team processes and promote a team environment

Minimum eligibility requirements * Successfully complete training as required or accepted by State regulations. * Must be 18 years of age or older. * Ability to read, write and speak English. * Ability to communicate effectively with residents, families, staff, vendors and the general public. * Be familiar with the Adult Care Home Residents' Bill of Rights and be willing to assist residents in exercising their rights. * Must have compassion for and desire to work with the elderly and understand that for each resident the facility is considered the Resident's home. * Must demonstrate the ability to work responsibly as a team member as well as an individual. * Must be honest, ethical, fair, dependable, respect confidentiality and the rights and privacy of others. * Must practice and promote Affinity Guidelines, Policies and Procedures, Mission Statement, values and goals. * Maintain practice certification and required hours of Alzheimer's and related dementia-specific training per year. * Exceptional organizational skills. * Must meet all health requirements. * Must pass criminal background check. * Must be able to perform duties and responsibilities (essential job functions), with or without reasonable accommodation. Essential functions Personal care and health care: * Assist residents with activities of daily living, including bathing, dressing, grooming (oral care), toileting, changing of bed linens, positioning, transfer, mobility, and incontinence care. Encourage residents to be as independent as capable. * Keep proper records of care as required by the facility. Complete all forms and documentation as required. * Immediately report any observations of concern/changes in resident's condition to the Supervisor in Charge. * Vital sign monitoring will be performed as needed or directed by regulations, policy or primary care provider's order. * Be able to apply the home's accident, disaster, fire safety, elopement and emergency procedures for protection of the residents. * Be familiar with Affinity's State Regulatory Policies and Procedures. * Be capable of performing CPR upon successful completion of CPR certification. * Be knowledgeable of the residents' background, preferences, habits and interests and preserve confidential information about each resident. * Focus on service to residents in during dining and promote meal time as an enjoyable activity of each resident's day. * Provide assistance to individual residents as needed or directed, who require help with eating, carrying plates, trays or beverages. Safety and Sanitation * Monitor all exit doors, the call and alarm systems to assure prompt response. * Understand fire watch and need to check exit doors during and after as directed to assure full closure and lock. * Maintain the confidentiality at all times of all mag-lock code systems to assure safety of the residents. * Promptly answer facility and common residents' telephone in a professional manner and per Affinity policy in order to meet the needs of the caller and communicate information. * Understand and follow safety and infection control policies and procedures and readily report any breach of the safety and infection control procedures to the Supervisor In Charge. * Respond to emergency call signals promptly. * Report incidents to the Supervisor in Charge promptly and complete appropriate paperwork as required in a professional manner. * Readily and directly report any verbal or physical abuse of a resident, inappropriate use of resident or facility property or lack of or inappropriate care of a resident to the Supervisor in Charge, RCM or Executive Director if necessary.
