Job Summary

Assists patients/residents/clients with therapy activities and exercises scheduled in a treatment plan, under the supervision of an occupational therapist, to improve patients'/residents'/clients' quality of life and ability to perform daily activities. Provides, monitors and records therapy services to patients/residents/clients with physical, cognitive, developmental and psychosocial impairments, in line with related standard concepts, practices and procedures.

Selects, implements, and makes modifications to therapeutic activities and interventions that are consistent with demonstrated competency levels, patient's/resident's/client's goals, and the requirements of the practice setting and/or state licensure. The certified occupational therapy assistant contributes to this process by exchanging information with and providing documentation to the occupational therapist about the patient's/resident's/client's responses to and communications during intervention. Responsible for being knowledgeable about the patient's/resident's/client's targeted occupational therapy outcomes and for providing information and documentation related to outcome achievement.

Follows and adheres to accepted clinical practice guidelines of professional organization. As needed, performs other related duties as requested.

Depending on location and facility needs, work may focus on those receiving intervention in the post-acute, long term care, and home health care environments.

Department Details

We are seeking an experienced COTA that can provide weekday coverage, Monday - Friday.


Appropriate education level required in accordance with state licensure.

Must be licensed as an Occupational Therapy Assistant in the state(s) of practice.

When working at a Sanford Health Facility (not required but preferred for GSS Facilities): Must also be certified as an Occupational Therapy Assistant through the National Board for Certification in Occupational Therapy (NBCOT). Conforms to the supervision and licensing standards of the state(s) where services are provided.

When applicable, may require valid driver's license.

Basic Life Support (BLS) certification required within six months of employment.

Position Summary: A Physical Therapist Assistant (PTA) works under the direction of a licensed Physical Therapist to help residents/patients with physical injuries or illnesses to regain range of movement and control their pain. PTAs follow plans created by physical therapists that involve stretching, strength training, exercises, and physical manipulation to help residents/patients recover. Note: All employees of Cascadia Healthcare are required to submit and be cleared to work in the facility per each states specific background check requirements prior to contact with patients/residents. Qualifications Education: * Graduate of an accredited Physical Therapy Assistant (PTA) program. Licenses/Certification: * Current state PTA licensure. * Valid CPR certification. Experience: * Six months experience in a long-term care environment preferred. Essential Functions * Helps residents with ambulation (walking, moving out of their wheelchairs, etc.). * Contributes to patients physical therapist's effectiveness by identifying patient care issues; providing information and commentary; recommending options and courses of action; implementing physical therapy directives. * Assesses patient health by interviewing patients; performing physical examinations; obtaining, updating, and studying therapy histories. * Assesses abnormal conditions by reviewing physical therapist's interpretations of patient evaluations and test results. * Documents patient care services by charting in patient and department records. * Performs therapeutic procedures by administering manual exercises; instructing, encouraging, and assisting residents/patients in performing physical activities, such as non-manual exercises, ambulatory functional activities, and daily-living activities; and in using supportive devices, such as crutches, canes, and prostheses. * Instructs and counsels patients by describing therapeutic regimens; giving normal growth and development information; promoting wellness and health maintenance. * Provides continuity of care implementing patient management plan. * Maintains safe and clean working environment by complying with procedures, rules, and regulations; adhering to infection-control policies and protocols. * Maintains production objectives by noting current treatment trends; monitoring actual physical therapy services rendered to production objectives * Ensures punctuality and regular attendance for assigned shifts.
