Premiums: Corrections Health Nurses receive an additional 10% hourly premium and hourly differentials for working swing ($2.50/hr), graveyard ($5.50/hr), weekends ($3.50/hr) shifts

Corrections Health provides medically needed health care to adults in custody of Multnomah County Jails and Juvenile Detention Facility, including the treatment of Opioid Use Disorder (OUD) with medications such as Suboxone (Buprenorphine/Naloxone). The Medication Supported Recovery (MSR), Licensed Practical Nurse (LPN) in Corrections Health is primarily tasked with the administration of MSR medications (Subxone, Subutex) to incarcerated patients under supervision of a Community Health Nurse (CHN) and supervisor. The MSR LPN also will assist with reviewing MSR referrals, and refer to a nurse or providers as appropriate.

This position is essential to the public health and safety, and required to report for duty regardless of County decisions to close or curtail other county operations. This position is located at the Justice Center.

Other duties include but are not limited to the following::

Medications-

Stock carts with prescription and other medications, removing expired medication from the carts, pushing heavy carts to assigned housing units, opening multiple heavy doors, identification of correct client, read medication administration records (MAR), pass out correct medication to the correct patient at the correct time, access clients on upper floors (stairs) as necessary, documenting medications. Identify untoward effects of medications that need follow up by a nurse. Other duties include working in the medication room in consultation with the CHN to manage incoming medication, order refills, and inventory and return medications to the pharmacy.

Panel Management and Referral Processing-

Utilizes electronic reports and review of provider schedules to proactively assess needs for prevention screening and communicates via EHR. Pro-actively performs pre-visit activities (e.g. scrubbing and huddling) for client visits, as needed.

Works with Providers and team to monitor chronic disease and prevention reports and initiates follow-up with clients or ensures appropriate referrals as needed. Forecasts immunizations. Leads the team and directs staff to complete duties, within scope, to support smooth clinic flow.

Review MSR referrals and start clients on MSR, based on criteria and established protocols. Schedule follow-up as appropriate and assist TSP CHN/RN with MSR referral follow-up and care coordination needs in the community. Collaborates with administrative staff to ensure appropriate tracking and follow-up of referrals.

Emergency Medical Response-

Responding to medical and custody emergencies by pushing emergency cart, perform CPR if needed, carry equipment up/down stairs, duties as directed by CHN in life threatening emergency situations.

Communicating Client Needs-

Reporting to and consulting with assigned CHN about any problems, adverse reactions from medications or unstable patients that the LPN may have identified during the medication rounds. Provides patient education.

Reviews cases with the providers and team as needed.

Works with RN to contribute to the plan of care. Follows an established plan of care. Seeks expert advice on issues for clients and provides input to providers and nurses on client response.

Patient Care-

Duties delegated by the CHN may including measuring and recording complete vital signs, blood sugars; checking dressings, answering phones, assisting with assistive devices, such as wheelchairs, stretchers, walkers; assisting with infirmary patients; such as personal grooming, dressing, bathing, feeding, transporting, lifting, turning, and other duties as assigned. Supports back office clinic flow by direct provider support as needed including rooming patients, ordering labs as directed.

Assists clinicians with procedures. Provides after visit instructions based on nursing experience and understanding of client needs; makes follow-up appointments, if necessary. Prepares patients for exams, and procedures, including equipment set-up and specimen collection.

Performs diagnostic tests such as labs, phlebotomy, EKG, etc. Completes Nurse standing orders per assignment. Maintains clinical skills as dictated by the client population and the clinical practice.

Completes competencies required for the position. Completes documentation and charting as required by policy.

Minimum Qualifications/Transferable Skills*:

  • Completion of an Oregon State Board of Nursing approved program at the certificate level or higher and a license to practice as a Licensed Practical Nurse as outlined by Oregon Revised Statutes (ORS) 678.021

  • Valid CPR/BLS certification upon hire.

  • Working knowledge of basic medication passing rules and protocols for patient safety, infection control and the limits of care governed by Medication Aide certifications.

  • Experience working in a Correctional Health Facility.
