The Bureau of Human Resources is accepting applications to establish a register to fill a vacancy located at the Maine Correctional Center.

JOB DESCRIPTION

Each resident is assigned a Correctional Care and Treatment Worker (CCTW, Case Manager) who serves as the primary case manager for their case plan. The position works with an interdisciplinary unit management team comprised of security, behavioral health and other specialized staff which assists each resident in the development of a case management plan and provides coaching to residents in support of their institutional adjustment and other needs.

The CCTW (Case Manager) is the primary staff person who works directly with the resident and other partners such as internal staff, community-based agencies, and Probation and Parole in the development of community reintegration plans. The position also offers non-clinical individual and group programming to residents in various domains, such as domestic violence, cognitive behavioral programs, and pro-social communication.

Typical Duties

  • Gather and review background information and conduct interviews in order to orient residents, obtain necessary information, and to conduct assessments.

  • Work with residents to develop and update case management plans, which includes appropriate referrals to programs, jobs, education and vocational programs and other interventions.

  • Track case-management-plan progress toward its goals and keep the interdisciplinary team informed.

  • Maintain thorough and up-to-date documentation in the form of case plans, progress notes, and other records.

  • Regularly meet with individual residents to provide support and coaching, and assist residents with obstacles, problem solving, goal development and reentry plans.

  • Collaborate with team members to modify case management and behavioral plans as needed to ensure safety and success.

  • Facilitate non-clinical programs and interventions with individuals and groups.

MINIMUM QUALIFICATIONS

In order to qualify, you must have

  • A three (3) year combination of education, training, and/or experience providing knowledge of case management principles and practices and/or correctional care and treatment programs.
