JOB SUMMARY: The Substance Abuse Education & Services Coordinator provides intake/counseling and addiction services in a congregate living, supportive housing program that is based on the Recovery Model to adults who have substance use disorders, and who are mentally and/or emotionally impaired and released from jail or prison.
ESSENTIAL DUTIES and RESPONSIBILITIES:
Provide initial assessment and drug screenings/intake and confidential addiction counseling, education, and support services to individuals, families, groups, and community promoting healthy lifestyles and healthy choices.
Assist individuals to Identify their need for treatment and create goals for treatment/aftercare plans.
Develop and present addiction and relapse prevention counseling to individuals and groups regarding the use and effect of alcohol and drugs, and relapse prevention. Maintain quality program practices (CQI) for developed addictions education program.
Collaborate with supportive housing team in identification of candidates who qualify for substance abuse services from the Department of Corrections, Federal Bureau of Prisons and the community.
Collaborate with other professionals involved with clients for individualized care.
Maintain strict confidentiality guidelines regarding client care.
Maintain accurate client records, and Individualized Goal Plans. Evaluate the effectiveness and progress toward treatment goals and communicate needs, progress, and barriers and report via the Efforts to Outcomes (ETO) Data Management System.
Attend continuing education courses as necessary.
Report progress and accomplishments to Office of Victim Services and Justice Grants (OVSJG), and Catholic Charities data reporting.
EDUCATION and EXPERIENCE:
Bachelor's degree from an accredited College or university in human services or related field.
3 years of experience in a social service organization.
Certified in Addictions Counseling (CAC).
SKILLS and COMPETENCIES:
Knowledge of client needs and risk assessments and service plans.
Knowledge of mental illness and addiction services, theory and recovery.
