Summary The Coordinator, Resident Services is responsible to coordinate the delivery of health care services, medical and behavioral monitoring, cognitive and rehabilitation modalities and other healthcare services within the personal care community in collaboration with all physicians, providers, network resourses and team members. Essential Duties and Responsibilities include the following:
Identifies and suggests innovative approaches and participates in implementing performance improvement opportunities. Maintains compliance with Presbyterian SeniorCare Network policy and procedure in accordance with governing laws, regulations, regulatory agencies, accreditation bodies and community standards. Demonstrates on a consistent basis individual and team behavior that supports person-centered, Woodwell and Woodside philosophies of Presbyterian SeniorCare Network.
Consistently upholds the mission, values and behavior standards of Presbyterian SeniorCare Network. Maintains confidentiality. Other duties may be assigned.
Communicates with resident, family, health care professional, peers and direct care team regarding residents health status to ensure coordination of care. Refers to appropriate health care agencies, i.e. home health and hospice, to ensure that health care needs are met by appropriate team members. Monitors resident record to ensure timely and appropriate documentation.
Completes all required documentation to ensure complianece with local and state regulations as they pertain to personal care. Particiatpes as an active members of the care management team to develop, update and utilize a person centered support plan that safeguards the residents rights, privacy and confidentiality while promoting the persons strengths and support of their care needs. In collaboration with the Administrator, responsible for personal care tean including monitoring and approval of the schedule and daily staffing assignments.
Conducts rounding and timely completion of annual performance evaluations, using opportunity to individualize personal and professional goals. In collaboration with the Administrator, develops and implements quality improvement process related to recruitment and retention of qualified team members. Serves and participates on Network committees as requested. # Qualifications To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Personal Care or Assisted Living experience preferred. Knowledgeable of DHS regulations. Computer Skills To perform this job successfully, an individual should have knowledge of Internet software; Spreadsheet software and Word Processing software. Certificates, Licenses, Registrations Valid current nursing License in good standing
