The Readmissions Coordinator is responsible for managing complex cases and serving as a knowledge liaison between acute and critical care teams. Direct all aspects of inpatient care, ensuring optimal length of stay, anticipate outpatient service needs, and set follow-up appointments, consultations as needed, and other components necessary in the discharge planning process.
Additionally, work closely with the Emergency Department Team to avoid readmission and address issues without having to incur an additional inpatient admission.
This position will also be responsible for reviewing and assessing all re-admissions identifying trends and areas of opportunity for improvement.
This is position is 5 days a week 8-hour shifts NewYork-Presbyterian Hudson Valley Hospital (located at Cortlandt Manor, Westchester County, NY), with the possibility of sporadic weekend and/or holiday requirements.
Coverage areas include Medicine units, Intensive Care units, Maternal Child Health, and Emergency Department.
Preferred Criteria
Prior experience in Care Coordination
Bilingual
Master's degree in nursing
Data collection and compiling
Required Criteria
NYS RN License
BSN
Three years' experience in clinical acute care nursing
Ability to function independently and communicate effectively with administrative and professional staff and with people of diverse backgrounds and education.
Ability to work with a variety of people and make decisions concerning issues involving discharge planning, home health situations and available community services.
Must possess good understanding of Managed Care/HMO requirements, laws and regulations, Medicare/Medicaid requirements, policy and procedure, and community health, welfare and social agencies policies and procedures.

Salary Range:
$124,000-$134,500/Annual
