The hospitalist nurse is responsible for collaboration with the entire medical care team to coordinate and manage the process of patient care from the point of admission through discharge. Integrates and coordinates clinical care of individuals, facilitates the interdisciplinary plan of care in order to meet multiple service needs, promotes continuity through elimination of fragmentation of care/service and facilitates the effective utilization of resources. Serves as educator and a central source of communication for the individual, their support systems, and the care team.

Responsibilities
Care Coordination
Triage of new patients: take calls from the ED/clinics/outside hospitals and assign new patients to providers
Round with providers, review charts, gather key items of clinical information for the provider and follow up on outstanding tests or treatments
Ensure provider attention to order sets, pathways, core measures, length of stay targets, etc.
Update white boards, answer patient/family questions, serve as a liaison between patient/family and provider, and deliver patient education
Coordinate with nursing, care coordination, and patient/family for discharge planning
Lead care coordination rounds
Assist with discharge preparation, including assisting with medication reconciliation and other paperwork, making post-discharge appointments, and pre-loading discharge instructions and orders
Field calls from discharged patients, outside pharmacies, mortuaries, etc. and fill out documents (death certificates, FMLA forms, hospice, home health, and DME requests) for physician signature
Follow up on tests resulted after discharge
Act on and attend corporate and local committees representing the Hospitalist Program
Collaboration
Collaborates with physicians, nursing, social work, and multiple disciplines, departments, payers, and agencies to eliminate barriers to efficient delivery of care in the appropriate setting
Coordinates and facilitates patient centered interdisciplinary care and communication, ensuring progress toward goal attainment
Supports and coaches accurate clinical documentation by physicians and other healthcare team members
Works effectively with multidisciplinary team assessing needs of the patient/family, establishing goals for the patient/family, and also assists in development of quality improvements that may cross over from department to department
Demonstrates the ability to be flexible in meeting needs of the department as a team member
Builds a network of positive working relationships that advocate for the patient
Delivery of Care
Delivers high quality care utilizing the nursing process, and demonstrates knowledge and clinical skills of the specialty required on assigned unit in accordance with established policies and procedures
Utilizes the Patient Centered Care model of care with the combination of Katarine Kolcaba's Comfort Theory
Provides skilled intervention for the support and/or resolution of patient and family crises, problem solving, and decision making
Improves patient and/or family understanding of and adjustment to the medical diagnosis and plan of care
Demonstrates a working knowledge of financial and reimbursement processes to facilitate medical cost management, including best practices, effective utilization of resources, linking clinical and financial aspects of care, and access to care and level of care
Utilize critical thinking in making independent judgments related to patient care. Maintain responsibility and accountability for knowledge of conditions as assigned patient populations.
Qualifications
Minimum Requirements

Education:
Must have completed an accredited nursing program
AD in Nursing, BSN preferred
Experience:
1 to 3 years clinical experience as an RN
License(s)/Certification(s):
Valid nursing license
Valid driver's license when driving any vehicle for work-related reasons.
Knowledge/Skills/Abilities:
Excellent communication skills, particularly in face-to-face personal contact with physicians, patients and families
Critical thinking and problem solving skills and coping effectively with stressful situations. Must possess time management skills
Understand the concepts of levels of care, payment systems and insurance requirements
Working knowledge of community information and resources for referrals
Knowledge and skills necessary to provide services/care appropriate to the age and diagnosis of patients served across the continuum of care, in accordance with the hospital/departmental/unit standards
Maintain working knowledge of applicable Federal, State, and local laws and regulations as well as other policies and procedures in order to ensure adherence in a manner that reflects honest and professional behavior
Discretion in handling highly confidential information from patients and ability to work with highly diverse populations
