Job Summary

Provides continuous observation of assigned patients, under the direction and supervision of a registered nurse, including verbally redirecting the patient from engaging in at-risk behaviors and summoning the nursing staff if the patient requires assistance.Job Description

Minimum Qualifications:

Education: High School diploma or equivalent GED.

Licenses/Certifications: Certified in Basic Life Support or Heartsaver CPR with AED within 90 days of employment.


  • Ability to understand verbal and written instruction and communicate clearly and effectively with hospital staff and respond quickly to patient behavioral changes.

  • Ability to perform duties requiring prolonged and/or extensive sitting, standing, and walking.

  • Ability to use discretion and courtesy when working with patients, visitors, and staff.

  • Provides continuous monitoring and observation of patients; coordinates with Registered Nurse on relief efforts when leaving the patient room in order to maintain constant patient observation.

  • Maintains visual observation of patient at all times; immediately summons the nursing staff if the patient requires assistance; seeks help or advice if patient appears to pose a threat to themselves or others.

  • Participates in a collaborative identification and reporting of patient safety issues, and ensures safe patient environment; seeks assistance with removal of any potential safety hazard from room and reports findings to the registered nurse.

  • Participates in hand-off of pertinent information/behavior about assigned patients when arriving on unit and upon completion of shift assignment.

  • As assigned by the PCA or other patient care provider, may assist patient with activities such as eating, cleaning, walking, brushing teeth/hair, etc.

  • Demonstrates safe work practices and attitudes; follows safety rules; works to prevent unsafe conditions and behaviors; and participates in organizational and department safety programs.

  • Considers the diverse needs of each patient, providing and ensuring the highest level of service according to established service standards, optimizing the patient experience.

  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.

  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.

  • Other duties as assigned.

PAC operates as a unit of Roper Industries, Inc The Director of Continuous Improvement will work with the executive management team to create and drive a strategy to be implemented throughout the organization. This role will provide functional leadership to management at all locations, and lead change across the organization through change management and continuous improvement strategies. Invest time and energy into the development of direct reports- lead annual performance development process, and recommend professional development & training as required. Lead all facets in driving continuous improvement by identifying and improving the outcomes of manufacturing processes. RESPONSIBILITIES Primary Responsibilities: * Facilitate in strategic planning and drive efficiency and effectiveness of enterprise capacity and resource management * Assist leadership team in achieving Strategic Plan objectives * Builds a CI culture across the organization * Drive continuous improvement to quantitatively and qualitatively improve costs, quality and service. * Partner with corporate and operations teams to create forecasting and future state plans, analyze cost trends and present to leadership team. * Drive Value stream mapping to define "As-Is-Analysis" and the transition to "Future State" * Perform internal and external benchmarking, analysis, target setting and publishing of key performance indicators (KPI's) * Leads and facilitates Kaizen events to drive sustainable improvements and works closely with plant personnel * Tracks and analyzes the CI process and maturity matrices * Support implementation of KPI team strategy and Daily Management tracking reward and recognize performance and execute on progressive discipline, as needed * Mentor, train and develop teammates for career progression and learning * Create a positive team dynamic that encourages all employees to: provide feedback and drive change within the operation, adapt to the ever-changing business and stay focused on the optimization of front end processes * Establish and mature the Lean Management System (tiered daily management systems, gemba walks, and leader standard work) and other coaching tools to transform the culture. * Develop/promote/standardize the CI culture. Share best practices and drive continuous improvement and Standard Work across all organizational elements. Additional Responsibilities: * Follow all company safety policies and procedures and organizes work areas for efficiency and safety. * Provides status reports to management as required. * Respond to change productively and handle other duties as required. Supervisory Responsibilities * Provides strategic direction, vision, and leadership to direct reports including coaching, mentoring and developing. * Conducts yearly performance evaluation in cooperation with functional leaders. * Confers with superiors, subordinates and other personnel to review achievements and discuss required changes in goals or objectives. QUALIFICATIONS Education & Experience: * 10+ years of progressive experience with demonstrated expertise and accomplishments in continuous improvement * Bachelor's degree, preferably in Engineering required * Advanced degree, preferred * Six Sigma Black Belt, required * Hands-on experience and demonstrated financial and operational impact through implementation of a variety of CI/LSS tools to drive Lean Conversion. Tools include 5S, Standard Work, Visual/Daily Management, Problem Solving Process, Kaizen facilitation, VSM, and Strategy Deployment * Demonstrated ability to communicate with personnel in all levels of the organization * Demonstrated ability to formulate strategy and gain buy-in from executive management * Experience must include implementing and managing systems on a corporate, divisional, or regional level with at least 5 years in a management or leadership capacity * Experience managing Continuous Improvement across multiple locations * Strong application knowledge of LEAN manufacturing techniques and related CI tools * Strong analytical skills, financial acumen, P&L Knowledge and ability to link improvements to financial and operational results * Business Acumen - Ability to understand interdependencies and the awareness to successfully think about and make the right decisions for the business.
