We are a Federally Qualified Health Center (FQHC) serving all those in need in the Sacramento area. We have been a level 3 Patient Centered Medical Home since 2011 and were the first to earn that designation in Sacramento. About the Position

This position is responsible for coordination and support of the One Community Health Quality Improvement Program. The incumbent assists in development and management of the Program in collaboration with the One Community Health Management team by helping to support the Quality Improvement Plan to ensure compliance with HRSA, state and local requirements. The CQI Coordinator serves as the resource for facilitating improvements in quality and services to One Community Health patients. Facilitates QI workgroups and other teams to address opportunities for improvements using recognized evidence based best practices for quality improvement. Develops reports to the Chief Information / Quality Officer. Conducts medical record reviews and submits clinical data to HRSA required quality studies via electronic mediums. Conducts staff training on improvement methods.

Essential Functions:

Assists in planning, development and management of the Quality Improvement program for One Community Health. Serves as expert resource for implementing and carry out of process improvements in all areas of One Community Health operations.

  • Assists in development, implementation, and execution of quality improvement plans for One Community Health.

  • Assists in development and implementation of One Community Health Quality Improvement Program policies and procedures.

  • Provides consultation to One Community Health committees, leaders and department managers to support quality improvement efforts being coordinated.

  • Assists One Community Health team members in interpreting and understanding performance improvement data and reports. Assists with preparation and distribution of reports/charts to leadership, providers and staff.

  • Provides educational support to One Community Health departments with regard to quality improvement and other regulatory standards as directed.

  • Assists in reviewing departmental indicators on a regular basis and makes recommendations as needed to ensure One Community Health Quality Improvement efforts are consistent with evidence based best practices.

  • Assists in the development of system for defining and monitoring departmental quality indicators.

  • Assists with ensuring that performance improvement activities are implemented and functioning in departments; provides training, support and reinforcement when workflows are not being followed; and reports back to supervisors as needed.

Education/Experience Required:

  • Nursing background preferred. (RN or LVN)

  • 1-3 years' experience in quality indicators including HEDIS and UDS

  • Clinical experience and / or quality care navigation experience is highly preferred.

  • Experience in health care related quality assessment and analysis

  • Ability to collect, correlate and analyze clinical data as appropriate to determine trends and develop interventions to address opportunities for improvement.

  • Possess excellent communication and presentation skills to deliver recommendations and findings to management and staff.

  • Strong communication and people skills necessary to deal effectively with problem solving, brainstorming sessions, strategy development and conflict resolution.

Essential Functions: * Assists in the coordination of the quality management/utilization management department activities with other medical center departments. * Acts as a consultant for appropriate departments on quality management, utilization management and risk management activities. * Assists with quality management studies through data collection, data input and report development. Provides objective assessment of important aspects of patient care through ongoing chart review with preestablished criteria. * Reports all occurrences which may lead to medical center liability and follows up as necessary. * Facilitates interdisciplinary collaboration for development of patient care paths. * Reviews hospital admissions and extended stays for specific hospital departments. * Serves as a member of the QM and/or UM Committees as needed. * Attends and/or participates in committee meetings as needed. * Assists in orientation and training of peers; in formulating and evaluating policies and procedures. * Performs special projects as assigned, such as statistical reports for state, federal, and local agencies. * Assists the director of quality and utilization management for QM and UM in the formulation of department goals and objectives. * Kaiser Permanente conducts compensation reviews of positions on a routine basis. Basic Qualifications: Experience * Minimum three (3) years of experience in acute care nursing as a RN. * Minimum one (1) year of experience in quality management. License, Certification, Registration * Current RN license in California. Preferred Qualifications: * Supervisory experience preferred. * Valid, current driver's license with personal means of transportation preferred. * Working knowledge of Microsoft Office applications preferred. * Experience as a trauma performance improvement nurse is highly preferred.
