Position Responsibilities:
The Consumer Directed Care (CDC) Coordinator is responsible for the initiation of the Consumer Directed Program (CDP) for consumers that are eligible for the program. The CDC Coordinator assists the consumer in understanding the process of being the employer of record. He/She also works as a liaison with the contracted Fiscal Intermediary, Northeast Arc, Care Managers, and GSSCs to ensure the highest communication is occurring in regards to the program.

Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)

  • Perform intake and orientation on new Consumer Directed Program referrals to enroll in the program and to instruct the consumer in the rules, policies, and regulations.

  • Provide functional skills training to instruct the Consumer/Surrogate in the basic requisites of an effective program of personal care services.

  • Instruct the Consumer/Surrogate on completing time sheets, understanding the medical issues and limitations of the Consumer and reviewing the responsibilities of the Consumer/Surrogate.

  • Instruct the Consumer/Surrogate on how to hire a new Consumer Directed worker, review paperwork for new hire and provide a new employee packet.

  • Keep current personal data, progress notes, service authorizations and tracking information in SAMS and case file.

  • Provide functional skills re-training to instruct the Consumer/Surrogate.

  • Addresses with Consumer/Surrogate any timesheet discrepancies and assists in resolving issues.

  • Completes, submits, and tracks all Consumer Directed Paperwork to Fiscal Intermediary.

  • Conducts CORI certification for new Consumer Directed workers.

  • Acts as a liaison with Fiscal Intermediary, Northeast Arc, Inc in regards to the Consumer Directed Services.

  • Tracks late timesheet submissions by consumers by reviewing monthly reports and following up with consumers as needed.

  • Lead in communication with all Primary Contacts within the agency who are affiliated and contracted for the Consumer Directed Program.

  • Other duties as assigned.

Qualifications:
BA/BS in social work, human services or related field required; experience with Elders in a Health Care setting required. Strong written, verbal, computer and organizations skills required. Ability to multitask and detail oriented a must. Bilingual ability in Spanish is required.

Hours: 37.5 per week

Primary Responsibilities: * Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care * Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services * Manage the care plan throughout the continuum of care as a single point of contact * Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members * Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team Expect to spend about 80% of your time in the field visiting our members in their homes or in long-term care facilities. You'll need to be flexible, adaptable and, above all, patient in all types of situations. Required Qualifications: * RN current, unrestricted in the state of Massachusetts * 4 years of experience working within the community health setting or in a health care role * Experience in long-term care, home health, hospice, public health or assisted living * Experience working with MS Word and Outlook * The ability to travel in assigned region to visit members in their homes and/or other settings, including community centers, hospitals or providers' offices Preferred Qualifications: * A background in managing populations with complex medical or behavioral needs * Experience with electronic charting * Experience with arranging community resources * Prior field based work experience * Bilingual in Spanish Careers at UnitedHealthcare Community & State.
