Under the general direction of the Clinical Lead and in accordance with standards of care and practice established by the Nursing Organization and the specific department; the rules, regulations and/or standards established by the State Board of Nursing, the Joint Commission, and appropriate specialty organizations relevant to the patient population under care, the incumbent performs the following functions: Essential Functions:

Patient Care Assessment: Interviews the patient and/or family to obtain a comprehensive history and to obtain data about patient's development status, behavior, cultural, and health concerns. Examines the patient to obtain objective physical data.

Reviews clinical data from diagnostic studies and previous records. Based on assessment, identifies nursing diagnoses/patient care problems. Documents data and assessment on appropriate form according to instructions.

Collaborates with the multidisciplinary team to address patient needs. Reassesses patient status at established time frames and as indicated by the patient's status. Reports status and changes in status as appropriate.

Assesses potential for development of crisis situations and asks for assistance. Performs telephone triage appropriately assessing signs and symptoms, potential risk for complications and necessity of immediate interventions. Identifies unstable or complex patient care situations and facilitates appropriate intervention with the Cardiologist/APRN, PA-C in the clinic setting.

Evaluates patients in the outpatient setting in conjunction with the Cardiologist/APRN, PA-C as needed. Completes data collection for Summit wide entities and reports percentage of readmits; develops objectives to achieve outcome measures and demonstrate the effectiveness of CHF Program. QUALIFICATIONS: Education:

Graduate of an accredited school of nursing. experience directly related to cardiology. Experience in performance improvement principles and group facilitation / process redesign principles, presentations required. Comprehensive knowledge of regulatory and accreditation standards required.

Knowledge base of cardiac terminology. Proficiency in Word, Excel, and PowerPoint required. Experience: Experience: 2 or more years of cardiology experience Strong knowledge of Congestive Heart Failure symptoms preferred Strong organizational skills Effective communication skills Licensure/Certification:

Current registration in the state of New Hampshire as a Registered Nurse. BLS certified. Must maintain BLS throughout employment.

ACLS certified. Must maintain ACLS throughout employment.

Provides patient-centered care with sensitivity and respects for the diversity of human experience. Performs technical skills as needed by specific clinical unit safely and competently. Coordinates and implements the medical diagnostic and therapeutic orders as directed. Anticipates and manages complications of disease progression and invasive procedures. Documents plan of care, assessments and interventions and patient outcomes in an electronic medical record thoroughly, accurately, and according to CMC Professional Nursing Practice Standards and ANA Scope and Standards of Practice. Reports status, including response to care, and changes in status as appropriate. Ensures calm, supportive and therapeutic environment for patients and their families, serving as an advocate when appropriate. Adheres to all safety, organizational and ethical standards according to ANA Code of Ethics. Implements elements of core measures in conjunction with Performance Improvement. Identifies current trends which could impact patient outcomes. Identifies system resources effectively and allocates them appropriately at the point of care delivery. QUALIFICATIONS: Education: • Graduate of an accredited school of nursing preferred. • BSN strongly preferred. • Specialty certification strongly preferred. Experience: • Minimum of one year of nursing experience and/or meets the requirements of Entry RN. Licensure/Certification: • Current registration in the State of NH as a Registered Nurse. • Basic Life Support (BLS) certification required. • Certifications per unit requirements: Refer to Education, Credentialing and Certification Policy.
