Concierge Primary Care Provider The Solidaritus Health Primary Care provider will be responsible for delivering broad-scope primary care services, coordinating comprehensive health care delivery for an assigned panel of patients, and nurturing a welcome and engaging health center environment. The Solidarius Health direct primary care model grants Primary Care providers with adequate time to: Listen to and care for patients Develop, adjust, and support patient adherence to individualized health care action plans Assist patients in adopting needed healthy behavioral change Prevent and manage chronic disease with support of interactive tools and processes Respond to patients' urgent and episodic sick care needs Provide ongoing healthcare education for Members and families Coordinate overall delivery of care across all care settings Develop a trusting provider-patient relationship with individuals and families under their care, becoming a trusted counselor and guide to Members in navigating the healthcare system and making key healthcare decisions Solidaritus Health eliminates the burden for virtually all medical billing, enabling the Provider to focus almost entirely on delivery of patient care. Solidaritus Health supports Provider success in meeting the above listed care delivery expectations by providing the following kinds of support: Tools, processes, peer collaboration, training, and mentoring Dissemination of best practices Flexibility and support for strategies to meet the health care needs of the assigned patient population A well-maintained and supplied Solidaritus Health Center to accommodate scheduled patient office visits and delivery of clinic-based care Digital communications tools to enable flexible remote delivery of primary care consultation to patients at locations and times that are mutually convenient for the Provider and the patient Consistent and competitive compensation Provider Responsibilities: Strategic Health Care Planning and Alignment Review health risk factors identified through medical claims data, health risk assessments, and comprehensive physical examinations of patients, and develop strategy for maintaining good health and improving patient health outcomes, increasing patient engagement (with high priority placed on engagement of at-risk patients), and improving patient satisfaction. Provide input and counsel on care delivery strategies, benefits design, incentive design, and health-related cultural change efforts. Effectively collaborate with peers, health center staff, and executive team members to ensure that Solidaritus Health’s values are implemented for the staff and patient experiences. Health Care Delivery Providers are empowered and expected to proactively deliver, coordinate, and manage overall health care for the population of Members or "patient panel" assigned to their care and to engage Members in their patient panels in self-management of their health. Specifically, Providers shall: Conduct comprehensive physical examinations and evaluations for patients and develop individualized Health Care Action Plans ("HCAP") that consider individual patient health needs and risk factors, as well as patient preferences, lifestyle, and goals. Utilize the Members' individual HCAPs as the framework for ongoing patient engagement and care delivered or coordinated by the Solidaritus Health Provider. Provide broad-scope primary care services to assigned members on the patient panel, including: Non-emergency, episodic sick care, urgent care, and emergent care triage chronic disease management Diagnosis and management of new health conditions Ordering and interpretation of diagnostic tests, including imaging exams, ECG's, spirometry, laboratory studies Prescription, dispensing and management of medications Performance of primary care appropriate medical procedures, including, but not limited to laceration repair and excisions/biopsies Complete accurate and thorough documentation of all patient encounters in the EMR system Implement appropriate medical protocols and clinical standards of excellence Provide support staff (medical assistants, LPNs, RNs, etc.) with clinical supervision and direction Participate in the identification of high value "downstream" provider partners (specialist and hospital centers of excellence) Develop relationships with high-value downstream providers to enable effective coordination and oversight of patient care when patients are referred for secondary or tertiary care Utilize available technology to ensure efficient management of patients Contribute to clinical improvements and efficiencies to benefit patient health and wellness, and streamline function in delivery of advanced primary care, both remotely and in the Solidaritus Health Center Provide effective care coordination for patients before and after hospitalization and any ER/urgent care visit Referral Management Coordinate patients' care across all settings, including transition back to primary care as soon as medically appropriate. Coordinate referrals to high-value providers and prepare records for specialists. In general, manage a "closed loop" referral process by referring patients to high-value downstream providers when clinically appropriate, conducting appropriate lab tests in the Solidaritus Health Center to the fullest extent possible, briefing the downstream provider on the specific purpose for the purpose for the referral, transmitting medical records, and coordinating care of referred patients as fully as possible to bring them back under principal care Solidaritus as soon as it is medically appropriate Required Qualifications: Experienced in either Internal Medicine or Family Medicine Experience with Adult and Pediatric primary care MD/DO - Board Certification in Family Medicine; or Board eligible – completion within 1 year of start date Valid and current license (MD, DO, NP) from an accredited program and DEA license A commitment to collaborating with the patient to improve their health through behavior change methods supported by Solidaritus Health A commitment to continuously improve the health and wellness of each eligible member Good leadership skills and communication skills Experience with electronic medical records A spirit of collaboration and cooperation with a willingness to continually learn and contribute to development and improvement of the Solidaritus Health direct primary care model and health programs Solidaritus Health offers our members Benefits: Health Insurance (family)
