Compensation:

• We offer a variable model that includes base salary plus performance driven bonuses.

• Base salary is factored on licensure level and geographic location paid on a biweekly basis.

• Social Worker license requiring supervision located in the state of Connecticut: $62,700 - $78,100

• Social Worker license that does not require supervision located in the state of Connecticut: $75,900 - $85,800

• Performance driven incentives can range from $860 to $2,200 based on data supportive targets paid on a monthly basis.

• Entry into the plan will begin on the first of the month following three months of completed service (month 4).

• You will receive a $500/month stipend for your first three months of employment.

• To ensure all employees are aware of their performance against their targets, and have full visibility into their variable compensation, regular 1:1 meetings will be required with their direct supervisor.

• Estimated Total Compensation: $68,000 - $112,200

Job Summary:

The Licensed Clinician is a core member of the Collaborative Care team involving the patient's primary care provider, psychiatric consultant, and other behavioral health providers. The Licensed Clinician provides evidence-based interventions using the Collaborative Care Model to promote system reduction and optimal patient outcomes. They use a world-class technology platform to provide brief interventions using evidence-based techniques such as Problem Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, and Behavioral Activation in Primary Care, Family Practice, and OBGYN settings.

Available Shifts in Eastern Time Zone: 8:00am

• 4:00pm, 9:00am

• 5:00pm, 10:00am

• 6:00pm, 11:00am

• 7:00pm or 12:00pm

• 8:00pm.

Required Education, Licensure, and Experience:

• Master's Degree in Social Work from an accredited program.

• Licensed to practice in their state as an independent social worker (LCSW, LICSW, or equivalent) OR

• Licensed to practice in their state as an associate social Worker under supervision and has passed the ASWB exam (LCSW, LMSW, or equivalent) AND

• Able to complete cross-licensing in Massachusetts within three months of employment. Concert Health will cover the cost.

• Minimum 1-5 years experience as a licensed professional.

Highly Preferred Experience and Skills:

• Bilingual in English and Spanish.

• Proficiency in evidence-based treatment approaches (Problem-Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, and Behavioral Activation)

• Expertise in handling higher-acuity patients/situations.

• Technical proficiency in a variety of platforms including EHRs, ability to quickly learn new technology.

• Knowledge of/experience with behavioral health screening tools (PHQ9, GAD7, CSSRS, etc).

Selection Process: Two Stages

• Qualified candidates will be scheduled for a 30-minute virtual interview to discuss experience, qualifications, base salary expectations, and an overview of Concert Health's Collaborative Care Model.

• Qualified candidates with the required experience, qualifications, and base salary expectations will be scheduled for a virtual interview with Clinical Leaders.

• Estimated completion time is two weeks.
