The Maternal Child Outreach program is a comprehensive prenatal services program providing services to women throughout Kern County. These services include prenatal care, delivery services, case management, one to one counseling, prenatal classes and educational, psychosocial, and nutritional assessments. The MCOP CPHW is part of the interdisciplinary care team that includes the health care provider, health educator, nurses, and nutritionists. This program also includes postpartum follow up at the hospital and in the clinical setting and family planning counseling.
1.Utilizes interview format to conduct prenatal and medical histories, and educational, psychosocial and nutritional assessments following established protocols. The CPHW provides support and health information as indicated by the assessments as well as referrals to community agencies and resources. Identifies high risk patients and refers according to established protocols.
2.Documents in the patient's medical chart and develops the care plan after the initial prenatal history.
3.Monitors patient's attendance to prenatal appointments, contacts patients to reschedule any missed appointments and documents in the patient's medical charts any contact made with patient.
4.Is able to be resource to the patient for problems and questions. Assists the patient in problem solving.
5.Teaches in prenatal classes following established outline and documents classes in patient's medical chart.
6.Conducts hospital visits and does bedside teaching on postpartum and newborn care. Schedules mother's six weeks and infant's newborn check appointments at the health center.
7.Conducts patients trimesters and postpartum assessments, completes assessment forms and up-dates care plan following CPSP guidelines.
8.Make referrals to appropriate community agencies and organizations as indicated by patients needs.
9.Counsels patients on all methods of birth control and charts in patients medical chart any counseling done.
Interacts with medical providers, nursing staff, nutritionist, and other team members for interdisciplinary team approach to patient's care.
Maintains patient files and completes data entry on patients.
Generated charges for services rendered.
Upholds and follows established policies on confidentiality and security.

1.Minimum 18 years of age.
2.Minimum: high school diploma or completion of a GED.
3.Basic computer skills.
4.Ability to meet deadlines.
5.Ability to multi task well.
6.Ability to work well with others and take direction.
7.Bilingual - read, write, and speak English and Spanish desirable.
8.Willing to be flexible with work schedule.
9.Valid California Drivers license and clean DMV.
Experience or desire to work with low-income families.
Full vaccination for NOVEL CORONAVIRUS (COVID-19) is required.
