Job Overview
The Manager of Complex Care is responsible for the successful development, growth, and strategic integration of the Transitions of Care, Clinician Home Visits, and Palliative Care programs within the clinic operations and in alignment with AltaMed's value-based primary care model. As an operational manager, this role collaborates with all stakeholders and cross-functional departments, including Specialty Services, Hospital Partners, Quality, Nursing, and Health Services to optimize patient experience while maximizing access, cost-effectiveness, and achievement of quality outcomes. Other responsibilities and accountability include but not limited to, the development and execution of financial, quality, service goals, objectives, initiatives, and action plans to achieve outcomes by identifying and monitoring key service line targets and metrics. Additionally, the manager leads or support the development and implementation of training, communication, and performance management of the Complex Care program staff to ensure all services are meeting or exceeding critical performance measures in health plan, compliance, strategic goals, and mission and vision of the corporation. Lastly, the Manager of Complex Care serves as the dyad operational partner to the Medical Director of High-Risk Visits on the strategic development and operational implementation of care models aimed at improving care for the transition of care and complex chronic disease patients.
Minimum Requirements:
1.Bachelor's Degree in Business, Health Administration, Nursing (RN), or other health or social service-related field required.
2.Master's Degree in Business, Public Health, Nursing (RN) or Health Administration preferred.
3.Minimum of 5 years of total experience in chronic disease care, primary care operations, palliative care/hospice, or a closely related field is required.
4.Minimum of 2 years in progressively responsible leadership or management role with experience in health care administration or closely related field required.
5.Minimum of 2 years of experience in operations management, program development, personnel management, project management, and quality management required.
