The Companion/Homemaker/Sitter is a non-certified non-professional worker who can perform the basic life skills necessary to keep a household functioning. May also provide companionship, monitor safety, and run errands or transport the clients to appointments and shopping.

1.High school diploma or GED preferred2. Basic life skills3. Informal experience with assisting the elderly preferred4. Desire to work with seniors and other adults with disabilities5. Able to communicate with clients/families and emergency systems personnel6. Able to work with minimal supervision

Position Responsibilities:

1.Works under the supervision of the Case Coordinator, Clinical Manager, or Agency Director with working hours being established on a case-by-case basis.2. Performs tasks only as assigned in a written care plan3. Performs routine household tasks as assigned in a written care plan including: a. Laundry b. Changing linens c. Dusting and vacuuming d. Cleaning the bathroom e. Cleaning the kitchen, doing dishes f. Taking out trash g. Other household chores as assigned that do not require strenuous effort (cleaning a closet, sweeping the porch, etc.) h. Run errands i. Meal preparation (grocery planning and shopping) j. NOTE: duties do not include heavy housework such as washing windows, moving furniture, other chores requiring climbing ladders, etc.4. Practices universal precautions, basic home safety, and utilizes good body mechanics5. Understands and can implement emergency protocols6. Provides companionship7. Maintains records as required:a. Documents tasks preformed with each visitb. Keeps accurate time records8. Notifies supervisor of significant changes to the client's situation, accidents, damage to the client's property, requests to perform services not included on the assignments, requests to change hours, etc.

Common patient populations include post-surgical (Cardiac, Orthopedic, GI, etc.), neurological, oncology, and general medical. The department has a culture of teamwork and collaboration. Staff are strongly supported by each other, as well as by leadership. Position description The patient care associate assists patients with delegated tasks of direct and indirect patient care, which support nursing practice under the direct supervision of a registered nurse. The patient care associate carries out designated activities that are within those functions limited by law to unlicensed nursing personnel. This includes assisting patients with activities of daily living, providing basic nursing care to patients and assisting in the maintenance of a safe and clean environment. Assists RN with procedures and treatments that require competence in advanced technical and manual skills. Qualifications High school graduate or equivalent within one year of hire. Additional qualifications Minimum of six months healthcare related experience preferred. Ability to read and communicate effectively in English. Flexibility (works flexible hours, which include days, evenings, nights, holidays, and weekends). Basic computer skills. Verbal and written communication. Promotes team building. Self-directed. Able to adapt to unpredictable situations. License or certification Certified Nursing Assistant on State of Wisconsin Nurse Aide Registry or will obtain prior to hire (required to be in "non-expired" status upon hire; recommended to regularly renew certification). Current Basic Life Support (BLS) Certification for the Healthcare Provider or CPR/AED for the Professional Rescuer or will obtain prior to hire. Exemption status Non-exempt Compensation Detail Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $14.65 per hour. Benefits eligible Yes Schedule Part Time Hours / Pay period 48 hours every two week pay period Schedule details 12-hour, primarily night shifts. May need to rotate to day shift as needed.
