Community Transition Supervisor

Community Solutions Inc. (CSI) is a nonprofit organization that promotes the responsible citizenship, accountability, and well-being of individuals and families involved, or at risk of involvement, in the child welfare, juvenile justice, or criminal justice system. Since CSIs founding in 1962, it has been a pioneer in the community-based service. We have been delivering supervision, treatment, and comprehensive individualized services to help clients interact more effectively in their communities and become productive citizens. Using this plan we built the first ever halfway house in New England which has now been replicated and grown to almost 50 programs in 9 states. For over 50 years, CSI has been building upon its successful foundations and has been expanding its services to serve thousands of adults, youth, and families each year.

CSI understands clients and staff have been impacted by trauma. CSI is becoming a trauma-informed organization by hiring individuals with existing experience in trauma-informed practices as well as supporting ongoing staff development with trauma-informed practices. This will include learning to recognize symptoms of trauma and how to respond, through training starting at orientation and throughout their career at CSI. CSI is committed to systematically evaluating the integration of the trauma-informed values of safety, trustworthiness, choice, collaboration, and empowerment at all levels of the organization's policies, procedures, and structures to guard against the possibility of re-traumatization.

PROGRAM OVERVIEW:

CSI is seeking a Community Transition Supervisor to lead our Community Transition Coordinators in our Connecticut DOC residential programs. The Community Transition Supervisor is responsible for ensuring all aspects of client discharge planning and community attachment to prosocial resources. The Community Transition Supervisor will keep the Program Directors updated on all contract adherence, staff performance, and program performance while guiding the Community Transition Coordinators to a successful operation of the program. The Community Transition Supervisor will complete the essential duties of a supervisor; hiring, training, coaching & development, assisting with evaluations, discipline and staff meetings. In addition to supervision duties, the position will serve as the CTC for his/her assigned program, performing all the CTC duties.

ESSENTIAL FUNCTIONS:

• Meets regularly with direct reports for supervision, coaching and development

• Recruits, trains, and assists with hiring, discipline, and evaluation

• Trains staff in keeping with the contract and role assignment

• Supervising and reviewing completed client discharge plans for submission to the Department of Correction

• Overseeing the coordination of all exit requirements with Probation and Parole Officers

• Participating in Prisoner Reentry Councils and Forums

• Assists in building a repository of community based services and resources

• Monitors to ensure follow through with referrals

• Collecting and synthesizing reporting meta data on services and performance metrics

• Conducting and collaborating in regular performance management meetings

• Conducting and collaborating with internal CTC team for regular case review and supervision

• Synthesizes post release follow up across programs

• Reporting post release data across programs

• Provides Quality Assurance review of client paperwork and electronic case management systems related to community transition

• Abides by agency confidentiality policies and applicable confidentiality statutes and regulations

• Maintains safety, appearance and cleanliness of CSI office and facility spaces

• Adheres to program procedures and enforces rules, regulation and policies

• Responds to staff and program crises and reports incidents to manager as detailed in Community Solutions, Inc. reporting policies and procedures

• Leads and/or assists and attends and participates in staff meetings, supervision, and on-going agency training

• Provides coverage as needed to ensure the safety and security of the facility

• Perform other duties as assigned by the supervisor.

QUALIFICATIONS:

• Bachelor's degree in a social science field and two years experience in social services or similar field. Education may be substituted with experience on a year for year basis.

• Supervisory background a plus

• Possess sound judgment and decision making skills.

• Software proficiency in Microsoft Office 365, Outlook, Excel, Word, email and video platforms such as Zoom and Microsoft Teams.

• Ability to work with spreadsheets and complex reports compiling and analyzing data

• Excellent oral, written and interpersonal skills desired.

• Experience with Microsoft Office.

• Valid driver's license and transportation
