Community Service Coordinator/Social Work Supervisor

Responsibilities:

  • Assists in securing and maintaining entitlements: SSI/SSD, PA, Medicaid, SNAP, Section 8 housing, etc.

  • Assists residents with the development and implementation of a long-range service plan

  • Conducts at least one monthly home visit and one phone contact with assigned residents and provides additional support to the resident(s) in crisis, as needed, and completes all appropriate documentation of each visit/contact

  • Maintains accurate phone logs (in addition to resident records)

  • Follows agency policies during all potential crisis/emergency situations

  • Documents of all incidents and occurrences

  • Conduct resident house meetings and fire drills as needed

  • Responsible for assisting residents at the time of moving in/out (ensuring cleanliness of all homes).

  • Responsible for contact and documentation of such with collateral service providers at least once a month and as needed

  • Reviews residents' rent status on a monthly basis

  • Participates in staff meetings and required training

  • Provides for the necessary development of ADL skills: budgeting, cooking, household maintenance, personal hygiene, use of public transportation, etc.

  • Fosters and maintains good relationships with neighbors and the general community

  • Assist in program admissions and discharge planning, as needed

  • Processes Homeless Housing Applications (referrals and screenings)

  • Participates in committees, activities and professional groups as required for the position

  • Reports housing maintenance issues through the appropriate procedure

  • Provide supervision of housing specialists and assist in staff training

  • Direct supervision of interns

  • Rotation of on-call duties

  • To assume additional responsibilities as needed

Qualifications

  • Master's Degree in Social Work or related Human Services field preferred OR

  • Bachelor's Degree in Social Work or related Human Services with experience with people living with mental illness required

  • CASAC and SIFI certification preferred

  • Supervisory Experience Required

  • Experience with people living with mental illness, MICA

  • Valid NYS Driver's License with a safe driving record, access to a personal and reliable vehicle

  • Physical Requirements: Sits, stands, stoops, squats, walks, and moves intermittently during work hours, may push or pull 10-15 pounds daily, may push or pull up to 50 pounds occasionally, drive agency vehicle

Our excellent benefits package and investment in our employees' success got us voted a Newsday Top Workplace on Long Island the past three years.</p><p>The wellness specialist is responsible for providing access to community resources, daily living skills training and support for those living in our supportive housing programs. The wellness specialist is a critical resource and advocate for our individual's.</p><p>Some of the responsibilities of the Wellness Specialist include:</p><ul><li><p>Facilitate individual's linkage and access to services available the community.
