We are currently seeking a passionate individual for the position of Community Services Specialist. In this impactful role, you will be instrumental in providing support to individuals within our supportive housing program. Some of the key responsibilities include:
Assisting in securing and maintaining entitlements such as SSI/SSD, PA, Medicaid, SNAP, Section 8 housing, etc.
Collaborating with residents on the development and implementation of long-range service plans
Conducting regular home visits and phone contacts, with additional crisis support as needed, ensuring thorough documentation
Establishing and maintaining contact with collateral service providers on a monthly basis and as required
Conducting bi-monthly fire drills at each residence
Ensuring cleanliness and setup of apartments for new residents
Focusing on the development of essential ADL (Activities of Daily Living) skills, including budgeting, cooking, household maintenance, personal hygiene, and use of public transportation
Assisting in the transportation of residents to Mercy Haven programs and events, including court and emergency medical appointments when necessary
Cultivating positive relationships with neighbors and the broader community
Participating in on-call duties on a rotational basis

Requirements
High School Diploma or Equivalency with experience in human services required
Bachelor's Degree in Human Services or related field preferred
Experience working with homeless families preferred
Valid NYS Driver's License with a safe driving record, access to a personal and reliable vehicle
Physical Requirements: Sits, stands, stoops, squats, walks, and moves intermittently during work hours, may push or pull 10-15 pounds daily, may push or pull up to 50 pounds occasionally, drive agency vehicle
Shift: Full-time (37.5 hours) Tuesday through Saturday (8:30 am to 4:30 pm), on call availability (Training takes place M-F)
