POSITION SUMMARY: Responds primarily to patients identified by SGMC Case Management that have been identified as at risk to be readmitted to the hospital.
The Community paramedic will assist with patient assessment and other needs to include but are not limited to performing 12 lead ECG's, lab draws, and administer medications.
The community paramedic will also assist with accessing living conditions and participate in an action plan specifically designed for the patient.
The community paramedic may also be asked to coordinate with other healthcare/public agencies for the patient's wellbeing.
The community paramedic will be responsible for the team's assigned vehicle and equipment ensuring that it is checked daily and that scheduled maintenance is performed per department's guidelines.
KNOWLEDGE, SKILL & ABILITIES: Currently licensed as a PARAMEDIC by the Georgia Department of Public Health Must be currently certified and maintain certification in BLS, Advanced Cardiac Life Support.
Must complete annual educational requirements as prescribed by Georgia laws / regulations, and department policies/procedures.
Must be able to demonstrate ability to perform practical skills proficiently in BLS, advanced cardiac life support, and other advanced life support procedures on adult, pediatric and infant patients.
Must have and maintain a valid driver's license and a good driving history in accordance with the Medical Center's insurance carrier's standards.
If not current, the following certifications are preferred: Emergency Pediatric Course; STABLE Program; Advance Medical Life Support; Advanced Stroke Life Support; Advanced Burn Life Support (if available); Haz-Mat Awareness.
NIMS ICS Programs 100, 200, 700, 800 are required within 3 months of employement.
If not current in PALS, must complete the certification within 12 months from hire date.
