ESSENTIAL DUTIES AND RESPONSIBILITIES
Under the general supervision of the Intake & Recruitment Manager, the Community Outreach Specialist will conduct networking activities to drive RiseBoro' s growth through referral source network building; develop lead-optimization strategies plan education; and help develop community engagement events to proliferate increased awareness, advocacy, and strategic partnerships with referral sources. This position will be focused on outreach to build strategic relationships with senior centers, physician groups, facilities, MLTC and CHHA contracts/contacts, community-based organizations, community leaders, faith-based organizations, social workers, and discharge planners. The Community Outreach Specialist is expected to maintain ethical conduct and will be responsible for the quality and satisfaction of prospective patients, referral sources, and contracts. Specific duties and responsibilities include:
Conduct outreach activities to promote awareness of the Homecare Program
Assist Intake Specialist in the areas of customer service, generating client outcome reports, and outreach analysis
Participate in outreach strategic sessions for the Home Care Program
Identify, develop, and implement outreach activities for the Homecare program
Coordinate all outreach efforts with Intake Specialist and Assistant Administrator; coordinate and attend events in the community, host events at our Best RiseBoro locations
Initiate and maintain relationships with community organizations, establishing linkage agreements and/or referral processes to the Homecare program; join network groups to create engagement opportunities with referral sources, prospective private patients/clients, and their caregivers
Communicate effectively with clients, patients, and their families, medical and community affiliates in order to develop positive relationships
Develop and facilitate presentations regarding topics that impact Short and Long-term Care for the client
Identify, attend, and promote the Homecare program at community meetings, Community events such as tenant associations, parent-teacher associations, and community fairs i.e., street fairs, health fairs, wellness fairs, etc.
Maintain a flexible schedule, including occasional weekend, early morning, and evening hours
Participate in team skillbuilding and professional development activities
Promote and maintain a supportive, positive, and welcoming environment, which reflects the mission, wisdom, and values of RiseBoro
Participate by working closely with the Intake Dept. & Recruitment Manager and department leadership
Collaborate with RiseBoro Human Resource Dept as needed for Homecare Worker referrals
Other related duties as assigned by the Administrator and Assistant Administrator
COMPETENCIES
Bilingual English/Spanish speaker required
Demonstrated related work experience in homecare, healthcare, and/or human services
Demonstrated ability to work with a diverse population, and contribute to building a positive team spirit
Excellent interpersonal and customer service skills
Excellent oral and written communication skills
Ability to effectively multi-task, follow up processes, and use independent judgment to produce quality work on time
Excellent organizational skills with attention to detail
Ability to be flexible and adaptable with work schedule and work tasks as needed
Proficient in Microsoft Office Suite; experience and working knowledge of homecare-related databases preferred
Ability to exercise good judgment and maintain confidentiality, particularly with client information

EDUCATION AND EXPERIENCE
High School Diploma or equivalent required
Associate's degree preferred
A minimum of one (1) year of related healthcare and/or human services work experience required; Homecare client enrollment-related work experience preferred
