Position Objective: Assist with providing administrative and program direction for Community Options program areas. To ensure all individuals have a safe home environment and/or respite care. Maintain the program in compliance with rules, regulations, laws, codes, and agency policies pertinent to each program area. To facilitate, develop, and implement training in accordance with Rule 50, Department of Human Services Rule 115, Rule 116, Rule 119, Rule 120, and other relevant regulatory guidelines. Will develop and implement individual plans and coordinate all habilitation direct care services provided by the facility to residents.

Essential Duties/Responsibilities

Mission Vision, Values

  • To support the philosophy through the agency's mission, vision, values and strategic plan.

  • Manage all responsibilities based on the agency mission, using individual choice and all appropriate policies, procedures, and regulations.

  • To conduct oneself in a manner that reflects the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts.

  • Coordinate and monitor functions of each area of facilities to ensure an efficient operation of the facility.

  • Monitor that all required documentation is completed regarding individuals served, quality assurance, and home environment.

  • Support Supported Living Program with the supervision of the Apartment Staff.

  • Coordinate and work close with the Supported Living Manager on any concerns or support regarding the SLA program.

  • Monitor relief hours/vacation per individual served with monthly summaries.

  • Provide OJT and CBTA training for DSPs when Host Home Manager requests support.

  • Provide on-going support and supervision to staff through monthly meetings, mentoring, and continuing education.

  • To monitor all host home providers complete drills in a timely manner.

  • Maintain files for Community Options to ensure compliance with rule 115 and rule 116.

  • Routinely complete quality assurance checklist of program, medical, and incident documentation to ensure consistent and thorough documentation is occurring, and report to Director any identified problems/issues in a timely manner.

  • As mandated reporter, ensure allegations of abuse and neglect are reported as required.

  • Participate in Community Options' (CO) management team.

  • Take minutes at all meetings and distribute to CO management team.

  • Submit recommendations regarding the development and/or revision of Agency policies, procedures and strategic goals pertinent to Host Home services.

  • Submit reports and statistical information in accordance with specified time frames as requested.

  • Interact with Community Options, Host Home providers, EP!C staff, Host Home Individuals, family members, other professionals, and other relevant individuals in a professional manner.

  • Ensure that home environments operate within established standards including health and safety guidelines, protection of individuals' rights, adherence to staffing patterns, and fiscal parameters.

  • Acts as a positive role model for individuals at all times.

  • Attend Community Options &/or Host meetings, in-services, as directed by the Community Option Director.

  • Assure compliance with all agency policies, licensing, accrediting, certifying, approving, and reviewing bodies affecting program operations.

  • Coordinate contractual agreements between providers and EP!C.

  • Contact Peoria area businesses and schedule educational/behavioral trainings for providers and/or individuals.

  • Participate in 24/7 on-call system

  • Perform other duties as assigned.

Assistance

  • Provide assistance to Assistant Director and Director in their absence.

  • Participate in agency-wide functions to support entire organization, including intra-agency and interagency committees and development of policies and procedures.

Staff Supervision

  • In conjunction with the HR department, interview, hire, supervise, assign, evaluate the performance of, and when necessary, discharge assigned personnel directly and in cooperation with appropriate department heads.

  • Authorize and plan personnel requests for vacation time, leaves of absence, travel authorizations, change in position and in-service training directly and in cooperation with appropriate department heads.

  • Assure all supervisory staff receive orientation, in-service and continuing education to increase the level of performance and maintain optimum standards of performance within the department.

  • Monitor all department staff to meet performance expectations and address concerns in a timely manner to assure peak performance for individuals served and other co-workers. Hold staff accountable for assigned responsibilities and meet monthly with each staff to review performance and develop a plan of correction as necessary.

  • Assure all host home and intermittent sites maintain appropriate paperwork for licensure and DHS regulations.

  • Monitor staff to assure host home providers meet all expectations of the contract.

  • Assure that all personnel are currently registered, licensed, and certified.

  • Meet with Director on a regular basis to ensure communication and review departmental information.

Program /Department Development, and Coordination

  • Meets all deadlines set by the agency and outside resources.

  • Review Relief summaries and ensure all documentation is completed to meet clinical and financial requirements.

  • Maintain files and data entry according to the standards set by the agency.

  • Review all documentation for accuracy

  • Understand rules governing program/licensure for CILA.

  • Supports database management systems.

  • Provide support to and work with all team and agency members.

  • Maintain positive parent and guardian relations for individuals on the team.

  • Promote community participation for individuals.

  • Review all medical and other incident reports for the person.

Marginal Responsibilities

  • Perform other duties as assigned.

  • As a mandated reporter ensures all allegations, abuse, and neglect are reported as required.

Education/Experience Requirements

  • One-year experience with persons with developmental disabilities.

  • A minimum of one-year supervisory experience.

  • High School Diploma or GED. Bachelor level Diploma preferred.

  • Valid Driver's License or State ID. Maintain an acceptable driving record.

  • Complete DSP training in accordance with the state regulations, and maintain status on the Health Care Worker's Registry.

  • Obtain/maintain standing as an Authorized Direct Support Person for medication administration.

  • Ability to lift 50 lbs.

Work Environment

  • Work may occur in the following settings: Home, Office, Site, or Community.

  • Participate in a 24 hour on call system.
