Provides basic medical care for patients experiencing homelessness as part of a new collaborative community program. Serves as the clinical liaison with Santa Barbara Cottage Hospital and community partners.

MAJOR ACCOUNTABILITIES

Clinical Practice Functions:

  • Triage patients presenting to the Santa Barbara Connect Home program.

  • Assess and intervene with patients per established hospital protocols.

  • Assess and refer patients as appropriate.

  • Performs initial assessments, patient education, coordination with the primary care provider, assisting patients with medications, and documenting and tracking patient medical needs and activities.

  • Provide clear documentation related to patient care and condition and maintains medical records.

Coordination and Collaboration Functions:

  • Collaborate with Santa Barbara Connect Home partners to conduct street outreach, including rounds with City Net, SB City Police Department, SB City Fire Department, and/or other partners.

  • Provide patient care management across the health care continuum including preventive care relevant to the patient.

  • Facilitate follow up appointments

  • Provide documentation for disability, insurance agencies, workers compensation, work releases and medical supply agencies.

  • Assist with the development and implementation of clinical practice guidelines.

  • Communicates with consultants as needed and serves as a patient and family liaison.

  • Create individual medical care plans for patients in the program.

Patient Education Functions:

  • Provide information to patient and patient's care team related to injuries, illness, prognosis and follow-up, and referral.

  • Identify high-risk groups and make appropriate referrals for Social Services and drug/alcohol programs.

  • Promote self-care in patients through education and support of self-care behaviors, counsel patients toward optimal physical, social, and psychological adjustment. Act as a patient advocate, supporter, and counselor.

QUALIFICATIONS

Associate degree in Nursing with preference to a Bachelor's degree in Nursing, Current, non-restricted California Registered Nurse license. Valid class "C" California Driver License. Verification of acceptable driving record from the California Department of Motor Vehicles.

Knowledge of advanced clinical standards, practices, and procedures for managing high-risk/at-risk patient populations; Knowledge of the normal signs of aging, including growth and development, and ability to provide age-appropriate care.

Other advanced knowledge of computers including EHR software, Excel, PowerPoint and other degrees or certifications useful in health-related fields.

A minimum of two years Previous work with patients experiencing homelessness or Psych nursing experience. Have an understanding that most illnesses and premature deaths are the results of lifestyle choices about diet, exercise, substance abuse, violence, and risk-taking behaviors.

Bilingual with fluency in Spanish is highly desireable

PathPoint is seeking a part-time Direct Support Professional to join our Community Access Project (CAP) team in Santa Barbara, CA. CAP is a state-of-the art program that provides skills training and support to adults with moderate to severe physical and developmental disabilities. As a Direct Support Professional, you will teach a variety of skills on community outings and in virtual and in-person classrooms. The fun curriculum includes art and music classes, recreation therapy (including yoga, stretching and bowling), cooking classes, adaptive technology, and more. The schedule for this position is Monday-Friday, 10:00am-3:00pm. Paid training and CPR/First Aid certification is provided. BA/BS in Psychology, Education, or related field is preferred. * Previous experience working with individuals with disabilities as a caregiver or DSP is desirable but not required. * Must pass pre-employment Live Scan DOJ fingerprint background check, TB screening and basic health screening. * Must have a valid driver's license with at least 5 years of driving history and a satisfactory driving record. * Ability to endure physical activities such as walking, standing, sitting, stooping, and lifting up to 50 pounds. * Must be comfortable assisting individuals with personal care/activities of daily living. Compensation and Benefit Package: $15.00-$15.40 per hour.
