Essential Job Functions: 1. Provides school-children's parents or guardians with information necessary to make informed consent for child's participation in the EOHSS Pilot Study.

2.Provides appropriate assistance to families on issues associated with program eligibility and appointment compliance to ensure assistance and questions are addressed timely.

3.Interacts with school-based healthcare and social service providers to facilitate scheduling appointments to ensure coordination of activities related to oral health screening activities.

4.Interacts with school-based healthcare and social service providers to facilitate distribution, completion, and receipt of parental informed consent forms in advance of scheduled screenings.

5.Confirms receipt of completed and signed parental informed consent forms prior to screening dates and ensures each school-child study participant presents with a completed and signed parental consent form.

6.Researches and responds to family concerns, outside agencies, and other third-party personnel to ensure questions/concerns are addressed in a timely manner.

7.Collects and scans all parental informed consent forms, maintains electronic file of all study associated parental consent forms.

8.Coordinates, distributes, monitors, and secures all iPads used for Electronic Oral Health Screening (EOHSS) activities. Administers surveys, ensures collection of data, ensures data is uploaded to HIPAA compliant database.

9.May assist school-based nurses with data entry into EOHSS Screening Instrument (iPad)



  1. Assists Project Evaluator with data collection.

  2. Travel between county schools and community locations.

  3. Performs other duties as assigned

Physical Requirements:

1.Near Vision

Job Requirements: Required Certifications/Licensures: Required Education: High School Diploma or Equivalent Major (if required): Required Experience: 1. High School and four (4) years of case management experience,

  • OR-

Associate degree or 60 college credits and two (2) years of case management experience,

  • OR-

Bachelor's degree and one (1) year of case management experience.

2.Ability to travel/drive between sites.

3.Computer skills such as excel, website navigation, and file transfers. Preferred Qualifications: 1. Experience with dental software programs.
