The Department of Population Health at the Dell Medical School is looking for a Community Health Worker.

This position is funded through March 31, 2025, with renewal based on funding availability, work performance, and programmatic progress. The Community Health Worker will be part of a multidisciplinary team, engaging vulnerable populations, facilitating health education, and coordinating care with health and social services providers. Responsibilities include connecting patients with unmet needs to social and medical resources and collecting data for health assessments.

Purpose

Work with a multidisciplinary team and community partners to help patients achieve improved health practices and increase health literacy to improve overall health outcomes. This role includes providing guidance and health assistance, coordinating service referrals, and delivering basic chronic health condition education. Additionally, it involves cultural mediation, offering social support, patient advocacy, and conducting outreach and community assessments. Providing community-based appropriate health education and care coordination is essential.

Responsibilities

  • Service Coordination, Navigation and Direct Service Skills: Connect patients to resources as needed, but not limited to medical homes and social services. Assist with follow-up appointments and care coordination. Organize and collaborate with strategic partners.

  • Education and Facilitation Skills: Use learner-centered teaching to provide one-on-one and or small group health education and encourage patients to adopt positive lifestyle changes. Schedule and or deliver health education classes in designated communities.

  • Individual and Community Assessment Skills; Outreach, Evaluation and Research Skills: Provide assessment and documentation of patients' health-related social needs and assets; participate in family and community assessments. Coordinate with team members across various project requirements. Assist with developing and implement assessment tools for project longevity and maximum positive patient outcomes.

  • Communication and Organizing Skills: In coordination with community partners, serve as liaison between Dell Medical School and other community partners. Utilize written and verbal communication, and assist with building a foundation for long term solutions.

  • Other related duties as assigned.

Required Qualifications

High school diploma or GED and 5 years of experience in community outreach, engagement, and delivery of health education, or a bachelor's degree in a relevant field and 1 year of experience in community outreach, engagement, and delivery of health education. Demonstrated experience working with community members, including youth and families, in various capacities. Proficient in interviewing techniques to gather personal information necessary for service enrollment, referrals, and resolving conflicting information. Strong knowledge of community-based resources and navigation, including health and social services, eligibility criteria, and how to access them. Bilingual in English and Spanish with the ability to communicate effectively and fluently in both languages, both verbally and in writing. Valid driver's license and access to a reliable car for travel within Central Texas. Relevant education and experience may be substituted as appropriate.

Preferred Qualifications

3+ years of demonstrated high-level interpersonal and relationship-building skills. Excellent verbal and written communication skills. Strong community engagement skills and a patient-centered mindset when working with internal and external partners. Demonstrated excellence in working with community members, including underserved populations. Experience working with local healthcare and community-based organizations. Proficient in using computer equipment and software, including Microsoft Office and Excel, with the ability to learn new programs. Bilingual in English and Spanish with fluency in both verbal and written communication. Knowledge of HIPAA/OSHA/ADA/FERPA compliance.

Salary Range

$43,000+ depending on qualifications

Working Conditions

Retirement Plan Eligibility:

The retirement plan for this position is Teacher Retirement System of Texas (TRS), subject to the position being at least 20 hours per week and at least 135 days in length.
