Trinity Health Mid-Atlantic is looking for an experienced Community Health Worker to join our team!

Schedule: Full time Day Shift (Monday - Friday) / Hybrid

This person will travel to various practices within the Philadelphia region.

A Community Health Partner (CHP) is a frontline public health worker who is a trusted member of and/or has a close understanding of the community served. This trusting relationship enables the CHP to serve as a liaison between healthcare/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The CHP also builds individual and community healthcare literacy and wellness by increasing health knowledge and disease self-management through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. The CHP assists in patient recruitment activities to insurance plans that will most effectively meet the needs of our population. The CHP possesses a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the mission, vision, and values of Mercy Health System and Trinity Health.

Responsibilities include, but not limited to:

  • Improves access to health and human services for patients through in-person interactions, telephone contacts, and coordination of referrals.

  • Facilitates or conducts enrollment in available federal, state and local programs to increase access to health care and support services. Examples include: health coverage via the marketplace, Medicaid, Medicare, social security benefits, food, and housing services.

  • Maintains frequent contact with community agencies serving vulnerable populations and participates in community meetings.

  • Assists members in their homes, community, or clinic setting. Communicates to clients/patients the purposes of the program and the impact it may have on their wellbeing. Helps patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.

  • Assists members in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and /or social services.

  • Continuously expands knowledge and understanding of community resources and services. Facilitates member access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and providers to teach life skills, relevant mental health services, etc.

  • Assists members in utilizing community services, including scheduling appointments with social services agencies and assisting with completion of applications for programs for which they may be eligible.

Minimum Qualifications:

  • High School Diploma or equivalent required

  • At least 1-3 years of healthcare experience preferably in the physician practice or community health setting

  • Previous MA experience a plus

  • Must be able to travel to physician practices and/or patient's home as needed.
