Under the supervision of the Community Engagement Program Manager the Community Health Worker Supervisor would assist to manage teams of Community Health Workers to assist with community populations to reduce barriers and offer health services through Public Health.

Management.Community Services:

  • Serve as lead point of contact to community health worker programs; ensure that necessary coordination takes place among operational managers, community team members, community outreach team members, site supervisors and community stakeholders, that results in cohesive communication and deployment of resources that meets regional leadership needs.

  • Supervise and direct Region Community Health Workers including approving time, scheduling, overseeing calendars, conduct regularly scheduled meetings and other tasks.

  • Actively support local staff to provide technical assistance in skills building to enhance capacity at the community level.

  • Assist team and work within assigned community to provide group class settings to help clients manage their own health including promoting self efficacy.

  • Assist patients in navigating the health care system; facilitate assistance or interventions for clients between care visits or programs; and collaborate with other care team members as needed to address issues outside scope of practice.

  • Develop strategic partnerships in the community to support community outreach and referrals; establish peer-to-peer relationships with community partners in order to function as a link to the community.

  • Facilitate linkages between community health workers and external partners to ensure technical skills and knowledge are appropriately available to meet community needs; stay current on community resources.

Emergency Preparedness:

In accordance with the State's Disaster Plan, which includes hazardous weather, DHEC employees may be required to work in times of an emergency or disaster.

This position is located within the Rock Hill Health Department located in York County.

Minimum and Additional Requirements

State Minimum Requirements: A bachelor's degree and relevant program experience.

Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.

Agency Additional Requirements:

Management Experience; a minimum of three (3) years experience as a Community Health Worker may substitute a bachelor's degree.

Preferred Qualifications

Community Health Worker Certification; and one (1) or more years of supervisory experience.

Additional Comments

This position is part of the South Carolina Department of Health and Environmental Control's Community Engagement. The work performed supports DHEC's mission to improve the quality of life for all South Carolinians by protecting and promoting the health of the public and the environment. Effective July 1, 2024, this position will transfer to the South Carolina Department of Public Health created by Senate Bill 399 which provides for the creation of the Department of Public Health to assume the public health-related functions of DHEC.

Driving Record: If this position requires the applicant to possess a valid driver's license to operate a state vehicle or personal vehicle, any applicant being considered in the final stages of selection for the position will be required to provide a certified copy of a 10-year driving record.
