Position Description

Performs advanced, specialized work to assist in the organization and implementation of various projects and programs. Work is performed under minimal supervision within established policies with latitude for independent judgment. Works on special projects as assigned. May substitute for supervisor during his/her absence.

Preferred Qualifications

Position Specific Qualifications:

  • Be certified Community Health Worker (CHW) or attending a CHW program that will complete within six months of employment

  • Ability to communicate effectively, both oral and written, with a variety of people

  • Ability to work collaboratively with others and to participate fully in an interprofessional team

  • Ability to understand and follow instructions. Work is performed in usual clinic and home conditions and requires a high degree of contact of both direct with patients and TTUHSC clinic staff

  • Ability to follow program and documentation requirements

  • Ability to receive instructions effectively; read and interpret documents such as reports, and departmental operating and procedure manuals

  • Ability to apply critical thinking skills, use common sense, and ability to carry out written oral or pictorial instructions

  • This position work schedule will include evening and weekend hours

Preferred Knowledge, Skills and Abilities:

  • Be certified Community Health Worker (CHW) or attending a CHW program that will complete within six months of employment

  • Must have ability to perform complex and diverse duties that involve application of procedures and demonstrate appropriate judgment within the CHW scope of practice

  • Knowledge in dealing with behavioral health issues and/or navigating the behavioral health system

  • Knowledge in developing relationships with individuals with serious mental illness and/or substance use disorder, and a demonstrated ability to connect with and support this population

  • Ability to increase health knowledge and self-sufficiency through a range of activities such as outreach, patient navigation and follow-up, community health education and information, informal counseling, social support, advocacy, and participation in clinical research

  • Ability to provide culturally competent support services for clients depending on individual needs and preferences

  • Ability to encourage healthy lifestyles for individuals, families, and communities through health promotion, outreach, and marketing

  • Ability to network/collaborate with community leaders, area health providers, community based organizations and others who can contribute to ongoing assessment of the health needs of the target population

  • Proficient typing skills; display accurate and skillful use of personal computer and

assigned software such as, but not limited to, Word, Excel, PowerPoint, Outlook Calendar, and Electronic Medical Records including the ability to meet documentation standards in a timely manner using the electronic medical record

  • Experience working in an office and outreach setting such as, scheduling and coordinating meetings

  • Working knowledge of HIPAA and maintaining confidentiality

  • Experience in data collection

  • Experience in conducting community referrals for clients

  • Community outreach experience

  • Excellent customer service skills

  • Excellent telephone etiquette

  • Excellent written and oral skills

  • Ability to communicate effectively in Spanish and English

  • Must have reliable transportation with current driver's license and current auto insurance

  • Other duties as assigned

Required Qualifications

High school plus six years experience in related field. Additional education may substitute for experience on a year for year basis.
