Program Description:

Public Health Solutions (PHS) is seeking a bilingual Community Health Worker for a HRSA-funded Healthy Start Program in South Queens. The HRSA-funded Healthy Start Program was created in partnership with Rising Ground (Healthy Families Program), Community Healthcare Network, and Jamaica Hospital Medical Center, with the goal of improving perinatal outcomes in the South Queens community through direct services such as home visiting and case management, group education workshops, doula support services, as well as community engagement activities through a formal Community Consortium that is specific to the Healthy Start model.

Summary of the Position:

The Community Health Worker (CHW) under the supervision of the Program Supervisor will work with low-income parents and their infants under 18 months of age residing in South Queens, including the neighborhoods of Jamaica, Howard Beach, Queens Village, and Far Rockaway.

Specifically, the Community Health Worker will:

  • Provide case management for up to 25 clients at any given time through one-on-one interactions with families including home visits, office visits, or community visits (i.e. meeting at a local library or park),

  • Identify and assist individuals and families to access needed preventive and primary health care services (e.g. preconception care, family planning, prenatal care, immunizations, pediatric care, breast feeding support, WIC and other nutrition services, inter-conception care, etc.), with an emphasis on identifying and supporting pregnant women not enrolled in health care or supportive services,

  • Provide assistance and/or referrals to obtain other essential support services such as emergency housing, public benefits, emergency food, baby items, immigration support, translation, fatherhood support and childcare,

  • Provide advocacy, support and follow-up to determine if services are received and assist families with health behavior changes,

  • Provide health education and parenting support using the Parents as Teachers curriculum, motivational interviewing skills, and individualized goal setting,

  • Conduct basic health and social assessments, assist families to identify needs, provide basic health information, and make appropriate referrals through weekly, biweekly or monthly home visits,

  • Maintain documentation of home visits, client interactions, and assessments using case management software, Well Family System Go-Beyond, contact referrals using Unite Us system

  • Conduct or assist in conducting educational group workshops and fatherhood groups,

  • Conduct community outreach (attending baby showers or other tabling events) on behalf of the Queens Healthy Start program as needed,

  • Perform other duties assigned.

Qualification and Requirements:

  • Associate's degree preferred, minimum requirement high school diploma/GED.

  • Experience working with families, migrants who reside in the shelter system in a community setting.

  • Ability to facilitate monthly fatherhood groups.

  • Ability to travel to different neighborhoods within Queens including Far Rockaway

  • Ability to be non-judgmental and establish trusting relationships; good problem solving, listening and practical skills; shows respect and sensitivity with individuals of differing cultural, racial, sexual or ethnic identities; some knowledge of community resources.

  • Open to working evening hours and weekends when necessary.

  • Bi-lingual preferred (Spanish and/or Haitian Creole)
