Sea Mar's network of services includes more than 90 medical, dental, and behavioral health clinics and a wide variety of nutritional, social, and educational services. We are recruiting for the following position(s):
Full-time Community Health Worker position available for the Care Management Program in Vancouver. This position requires that applicant travel to throughout the Clark County sites. Applicant must have access to an insured vehicle, and be able to make home visits.
This position requires flexibility, initiative, and the ability to manage time, resources, and client caseload efficiently. Under the direction of an RN & Social Worker, may follow up on clients in their homes and deliver basic health messages, and ensure that utilization of medical care and other community resources are met. This Transition of Care Program (TOC) works with inpatients to prevent unnecessary readmissions.
Qualifications and/or Education:
Bachelor of Arts required.
One year of health and/or social services experience preferred.
Bilingual English/Spanish preferred.
