CHWs (Community Health Workers) are individuals that love to help in their communities with a natural gift for connecting with people.  You are not afraid to roll your sleeves up to drastically improve lives in their communities, as you have a strong sense that something is not right in healthcare, and you want to help make it better.  Matter’s Community Health Worker is excited to be a part of the team, working to champion our mission among older adults in our Knoxville market.  Our CHWs are driven, creative, organized, flexible and detail-oriented with great problem-solving skills. They have a devotion to working with the community and willingness to learn in order to serve our patients.  

The CHW will work closely with patients to support care coordination to address social needs. Through this process, they will assess the social needs of Matter Health members and connect them with the resources needed to impact health. They will work closely with Matter Health’s clinic care teams, support staff, and residential property management to plan and carry out outreach and engagement activities that promote health and wellness and connect older adults to services, resources, and information. As appropriate, they will work cohesively in a team environment with other CHWs to plan and implement community programs that support the overall health of the members. 

  • Develop and co-facilitate group activities (i.e., classes, support groups, workshops) and provide health education and information to low-income older adults and their families 
  • Conduct community activities/events in various forms (e.g., in person, virtually, door-to-door, one-on-one and group activities)  
  • Participate in planning, execution, and follow-up for large quarterly events at each of the properties within assigned market.     
  • Ensure client database is always well-maintained, making sure to include all necessary notes, forms and supplementary information through weekly checks and updates to promote efficiency. 
  • Identify social needs and manages referrals and resources to social services to meet members overall personalized goals,  
  • Develop and maintain community partners and resource directory to ensure members needs can be met 
  • Engage with and document social care in appropriate systems  
  • Focus on the ongoing development of trust and fostering great relationships with the residents and property staff 
  • Make referrals to appropriate health services and resources to ensure residents and members receive the help 
  • Assist residents with filling out forms related to services sought/rendered (I.e., SNAP, Medicare, Medicaid, VA Benefits) 
  • Participate in opportunities to work with other institutions and organizations to meet community needs and make recommendations for improved services 
  • Maintain high level of confidentiality of patients and comply with HIPAA (Health Insurance Portability & Accountability Act) regulations.  
  • Other duties as assigned 

Requirements

  • Passion for Matter Health model, vision, and patient population  
  • An Associate degree in health and human services, social services, public health, or four years of relevant experience, or equivalent work related experience.
  • 2 years' experience working/volunteering at community- based organizations or non-profit organizations 
  • 2 years' experience working with the older adult population 
  • Ability to use electronic devices such as Google Chromebook and tablets 
  • A strong understanding of culture and traditions of the Knoxville community 
  • Active unrestricted driver’s license and a clean driving record  
  • Reliable vehicle with current registration and state minimum insurance 
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. 
  • Working knowledge of community-based programs related to health care delivery, clinical education, and health-related services.  
  • Ability to plan, implement, and evaluate programs, initiatives, and events. 
  • Skill in use of personal computers, iPads/iPhones and related software applications, and the Microsoft Office Suite 
  • Strong written and verbal communication skills.  
  • Ability to work effectively at all levels in a collaborative, team oriented and Patient’s first environment.
