As a Healthcare Services Coordinator, you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education.

Primary Responsibilities:

  • Create a positive experience and relationship with the member

  • Keep the member out of the hospital by supporting regular visits to their primary physician

  • Keep member actively engaged with their primary physician

  • Support the member to ensure pick-up of their Rx

  • Proactively engage the member to manage their care

  • Provide member education

  • Support transitions of care

  • Help to keep members compliant with their care plans

  • Partner with care team (community, providers, internal staff)

  • Knowledge and continued learning of community cultures and values

  • Traveling to member homes and medical facilities such as a hospitals

  • May conduct HRA assessments if needed

Required Qualifications:

  • High School Diploma / GED (or higher)

  • Ability to travel up to 75% of the time for field based work

  • Ability to work 40 hours 8am - 5pm Monday-Friday

  • Must have resided within the community for 2 years

  • Knowledge of culture and values of community

  • Familiarity with the resources available in the community

  • Field based experience

  • Ability to navigate a PC to open applications, send emails, and conduct data entry

  • Must live within King County, WA

  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area

Preferred Qualifications:

  • Associates Degree (or higher) in social work and / or health care administration

  • Ability to create, edit, copy, send and save documents, correspondence, and spreadsheets in Microsoft Excel, Microsoft PowerPoint, Microsoft Word, and Microsoft Outlook

  • Licensed practical nurse (LPN), Certified Nursing Assistant/Home Health Aide, Medical Assistant

  • Bilingual in Spanish and English

  • CHW accreditation

  • Experience working in managed care

  • Knowledge of Medicaid population

The Child and Family Therapist works with youth and their families providing support and assisting them in building the parent/youth communication skills, assessing and enhancing the safety and structure within the home. This position works as a part of a comprehensive team which includes a Parent Partner and Youth Peer/Outreach Worker who utilize their own experiences to support families as well as helping them find resources and natural supports for long term stabilization within this short-term model. An ideal candidate has the ability to work independently and as a part of a team. Experience delivering evidenced based programs within the community a plus. We are looking for individuals that are skilled at working cross-culturally and demonstrate an awareness of cultural diversity and the impact that trauma has on youth and families. GENERAL FUNCTION Provides comprehensive short-term mental health services to youth and their families within South King County. Collaborate with a team to support families in finding ways to be connected and safe. ResponsibilitiesUtilizes evidence based treatment modalities to provide individual and family stabilization services, including crisis prevention and safety planning, parenting skills training, behavior management, de-escalation training and brief counseling and case management.Maintains a caseload of assigned clients. Assists and partners with clients to succeed in school, job, home, courts, and community settings.Develops and implements individual Service plans in partnership with the individual and family that are sensitive to cultural, social, mental health and recovery needs.Responds to emergencies in a timely and effective manner during regular hours.Accurately completes all required paperwork in a timely fashion.Teaches families how to facilitate their own recovery, access support groups, utilize effective problem solving techniques, and identify natural and community supports that are conducive to recovery.Collaborates effectively with other professionals including attending Family Team Decision Meetings, Child and Family Team meetings, Wraparound Team Meetings etc. in order to meet the goals of the individuals and families.
