You must live in North Carolina.

This position requires use of your own vehicle, a valid drivers license, and car insurance. For site visits to remote locations travel of up to 100 miles (you will be paid a mileage reimbursement) per day may be required. There will also be days where no travel is required. We anticipate the site visit outreach (travel) to be 3-4 days per week, with work from home 1-2 days per week.

Who we need:

Kepro is looking to recruit Community Health Workers to make outreach to persons who have been tested for COVID. The role of the Community Health Worker will connect persons with COVID 19 to resources including testing/contact tracing, primary care and related support services through telephonic outreach or face to face encounters. The care coordination outreach activities are designed to support individuals impacted by COVID 19 and reduce short and long term effects of the illnesses.

What youll do:

  • Ensures the responsible delivery of comprehensive services to enrollees.
  • Conducts general assessments and provide education and support based on individual needs.
  • Increase health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
  • Participates in the interdisciplinary case reviews for collaborative assessment and coordination planning to ensure quality care.
  • Assists Lead, Supervisor and/or Manager in ensuring achievement of contractual financial obligations, including service delivery in a cost effective and efficient manner and through support of budgetary adherence by reporting accurate and timely work hours and expenses incurred during course of position duties.
  • 2-4 years years experience in care coordination or field environment, preferred
  • This is a remote position that requires good internet connect and a quiet work space.
  • Attendance at in-person events to provide education and direction and assistance with vaccination events.(PPE is provided.)

  • Associates degree (or higher) in health related field and/or medical assistant training or higher, strongly desired
  • Bi-lingual (Spanish) highly desired.
