Community Health Worker

POSITION SUMMARY: The Community Health Worker (CHW) will be responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports providers and the Chronic Care Management Coordinator(s) through an integrated approach to chronic care management, community outreach, coordination of services and follow-up. The CHW will actively promote, maintain, and improve the health of patients and their family through social support and informal counseling, advocate for individuals and community health needs, and provide services (i.e. blood pressure and blood glucose screening, etc.). Promote wellness by providing culturally appropriate health information to patients/families and care teams, education patients on disease prevention, conduct health related screenings and facilitate with other support groups.

• High School Diploma or equivalent

• California Driver's License

• CPR *American Heart Association

• Knowledge of Microsoft software (Word, Excel)

• Must be able to speak English and Spanish

Position Pay Range: $16.00- $25.18/hour
