POSITION SUMMARY

The Community Health Worker (CHW) reports to the Director of Ambulatory Services and the Social & Community Services Supervisor and is responsible for assisting patients and their families navigate and access community services and other resources. The CHW supports Providers and the Care Management through an integrated approach to care management and community outreach. The CHW will provide social support, resources and community outreach activities to a underserved population of patients in the outpatient setting. Services will be provided to individuals that come from diverse cultural backgrounds, ensuring that underserved communities receive the proper resources.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • In collaboration with the Social Worker/Social Services Specialist will conduct comprehensive screenings for patients.

  • Establishes rapport with patients and their families while providing general support and encouragement for services.

  • Comfortable in working with patients who are considered vulnerable, challenging to engage, or those who suffer from mental health and/or substance abuse concerns.

  • In collaboration with the Social Services Specialist/Social Worker and community agencies will provide education and resources to patients and/or their families driven by an individualized plan of care that is culturally appropriate.

  • Works collaboratively with other clinical personnel assigned to the same patient.

  • Knowledgeable about community resources appropriate to needs of patients/families.

  • In collaboration and guidance of the Interdisciplinary Team, provide referrals and coordinates services to community agencies as appropriate with a warm hand-off.

  • Responsible for providing consistent communication to the Interdisciplinary Team.

  • Follow-up with patients via phone calls and visits to other settings where patients are referred (shelters, group homes, recuperative care facilities), including the gathering of data regarding services accessed.

  • Acts as a patient advocate and liaison between the patient/family and providers and community service agencies.

  • Documents patient care management information in the EMR (training provided) and other software

  • Assists with and manages assigned caseload of patients.

  • Other duties as assigned.

POSITION REQUIREMENTS

A. Education

  • Bachelor's degree in Social Work, Public Health, Psychology, Sociology, Human Services, or related degree required. Candidates with at least five (5) years of experience in a similar role will be considered in lieu of degree.

B. Qualifications/Experience

  • Bilingual Spanish preferred.

  • At least one (1) year of experience in Care Coordinator and/or Community Health work.

  • Basic Life Support (BLS) Required.

  • Knowledge of medical terminology preferred.

  • Experience working in a multi-cultural community based setting for at least 1 year preferred

  • Microsoft Office experience

C. Special Skills/Knowledge

  • Written and verbal fluency in English required. Good communication skills and willingness to provide emotional support, encouragement and motivation to patients.
