Be a part of our Chronic Disease Prevention Program (CDPP), where under the general supervision of a Physician, Advanced Practice Provider, Nurse, Social Worker or Health Educator, you'll have the opportunity to assist in providing community outreach, education, investigation, case coordination and assistance. The CDPP implements and administers prevention and education programs that empower people to prevent disease, improve health and promote wellness. The program offers diabetes prevention and Chronic Disease Self-Management education; diabetes, obesity and hypertension health risk screening; support to resident-led walking clubs; free fitness and Tai Chi classes, and information, resources and referrals at community events.

WHAT WE OFFER:

The County of Ventura offers an attractive compensation and benefits package. Aside from our base salary range, an employee within this position will also be eligible for the following:

  • Educational/Bilingual Incentive:Incumbents may be eligible for an educational incentive of 2.5%, 3.5%, or 5% based on completion of an Associate's, Bachelor's, or Master's degree. Incumbents may also be eligible for bilingual incentive depending upon operational need and certification of skill.

  • Health Plans:Biweekly County contribution towards medical, dental, and/or vision insurance from a group of authorized plans

  • Deferred Compensation-Eligible to participate in the County's 401(k) Shared Savings Plan and/or the Section 457 Plan.If you participate in the County sponsored 401(k) plan, the County will match a part of your contribution up to 3% of your salary, on a per pay period basis. Following five (5) years of continuous County service, an additional 2% non-elective contribution will be made to the 401(k) plan.

  • Pension Plan-Participation in the County's defined benefit pension plan. If eligible, you may establish reciprocity with other public retirement systems such as CalPERS.

  • Holidays-12 paid days per year which includes a scheduled floating holiday.

Community Health Worker is represented by the Service Employees' International Union (SEIU) and is eligible for overtime compensation.

The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-Term), Intermittent, and Extra-Help vacancies for this position only.There is currently one (1) Regular vacancy within the Chronic Disease Prevention department.

Examples Of Duties

Duties may include but are not limited to the following:

  • Patient/Client Intake: assists in the initial patient/client assessments and follow up including basic intake interviews and screenings; assists individuals or families to define problems and needs, and place them in order of priority;

  • Assist in Implementing Care Plan: Interpret and monitor patient/client compliance; provide and reinforce health education and Care Plan; help patients/clients make appointments and provide assistance to ensure appointments are kept; report observations and finding to nurse or clinician;

  • Keep Appropriate Records: document client encounters and progress notes consistent with agency and program requirements; document findings of individual encounters and focus groups and community interactions;

  • Provide Health Education: using standardized or approved curricula, through the Centers for Disease Control and Prevention (CDC) for the Diabetes Prevention Program, provide basic information verbally, written and with visual aids; teach basic health, living skills and home safety;

  • Addresses barriers to care by linking patients/clients to community resources and provides care management services;

  • Assists patients/clients enroll in programs that support preventative health and treatment;

  • Follows program guidelines and regulations, including eligibility and billing;

  • Meets productivity and quality goals set by agency;

  • Participates in care team huddles, meetings, and required training;

  • Perform Community Outreach: acts as Department liaison with community groups and clients and informs them about community services; conduct focus groups in the community on health and social problems; conducts biometric screenings, which include finger pricks, blood pressure, height, weight, and BMI measurements;

  • Advocacy: help clients and groups identify their unique strengths in meeting difficult health and social issues; helps and provides support to clients and groups in identifying, developing and implementing action plans that help address family and neighborhood issues; seek ways to provide public and private acknowledgment to clients and groups implementing health care changes; and

  • Perform other duties as assigned.

Typical Qualifications

These are entrance requirements to the examination process and assure neither continuance in the process nor placement on an eligible list.

EDUCATION, TRAINING, and EXPERIENCE:

High school diploma, GED or equivalent, plus any combination of education and experience which has lead to the acquisition of the required knowledge and abilities.

NECESSARY SPECIAL REQUIREMENTS:

  • Must be bilingual (English/Spanish)

  • Certificate of completion for the Lifestyle Coach for the National Diabetes Prevention Program.

  • Intermediate experience and ability to work in a Windows environment and using Microsoft Word

DESIRED:

  • College course work in social sciences, ethnic studies, health sciences, communications, or related fields

  • Investigation related experience

  • Possession of a Certified Phlebotomy Technician I (CPT I) Certificate from one of the certifying organizations approved by the California Department of Public Health

  • Certified in Chronic Disases Self-Management Programs through the Self-Management Resource Center or by a certified master trainer.

  • Experience working in Ventura County's Medical Center or Public Health Department

KNOWLEDGE, SKILLS, and ABILITIES:

Some to thorough Knowledge of:

  • community service organizations that provide services to the agency's target population; medical terminology;

General knowledge of:

  • community agency program policies and procedures;

group and family dynamics;

  • preventative health maintenance; prevention of type II diabetes;

  • Medical guidelines;

Skills in:

  • computer literate in Windows environment and skilled in use of Microsoft Word;
  • ability to develop trusting and effective working relationships with a diverse client population;
  • able to lead and/or work effectively in a team environment;

Ability to:

  • effectively communicate with individuals and groups from diverse cultural, socio-economic backgrounds;
  • teach and follow up on chronic disease and diabetes prevention with clients;
  • maintain case management notes on computer system;
  • perform various health screening tests such as blood pressure, BMI, weight, height measurement and finger pricks;
  • monitor client compliance with health care plan;
  • accurately collect large amounts of data that are used for reporting and billing purposes.

WRITTEN EXAMINATION - 100%:An online, un-proctored job-related written examination will be administered to test candidates' knowledge of language usage, interviews/confidentiality, medical knowledge and terminology, interpersonal relations, and filing. Candidates must earn a score of seventy percent (70%) or higher to qualify for placement on the eligible list.

Applicants successfully completing the examination process may be placed on an eligible list for a period of one (1) year.
